    Impact of Inspiratory Muscle Strength and Lung Function on Functional Exercise Capacity in Post-Myocardial Infarction Patients: A Cross-Sectional Study

    OBJECTIVE: This study aims to investigate the contribution of lung function and respiratory muscle strength in predicting functional exercise capacity in post-myocardial infarction (MI) subjects. MATERIAL AND METHODS: This cross-sectional study included 56 stable post-MI subjects. Lung function was assessed using a digital spirometer, and respiratory muscle strength was measured using an intraoral pressure meter. The 6-minute walk distance (6MWD) was conducted to assess functional exercise capacity. Correlations and multiple regression analyses were performed to evaluate predictors of 6MWD, considering demographic factors, lung function, and respiratory muscle strength. The Bland-Altman plot was used to investigate the agreement between observed and predicted 6MWDs. RESULTS: Significant positive correlations were found between 6MWD and forced vital capacity (FVC)%predicted (r = 0.528, P = 0.022) and maximum inspiratory pressure (MIP)%predicted (r = 0.640, P = 0.022). Age (r = -0.350, P = 0.008) and body mass index (BMI) (r= -0.561, P < 0.001) were negatively correlated with 6MWD. The best regression model included MIP%predicted (β = 0.332, P = 0.002), BMI (β = -0.264, P = 0.012), being male (β = 0.262, P = 0.003), age (β = -0.210, P = 0.020), and FVC%predicted (β = 0.219, P = 0.026) as significant unique contributors. The final multiple linear regression model was significant [F (5, 50) = 19.08, P < 0.001] and explained 65.6% of the variance (R2 = 0.656) in the 6MWD. CONCLUSION: Lung function and respiratory muscle strength significantly contribute to functional exercise capacity in post-MI. This study emphasizes the importance of comprehensive respiratory function assessments in rehabilitation strategies to improve exercise capacity in patients with post-MI.

    Aile Hekimlerinin Sağlıklı Yaşam Biçimi Davranışlarının ve Klinik Uygulamalarında Yaşam Biçimi Tıbbının Değerlendirilmesi

    Amaç: Bu çalışmanın amacı, aile hekimlerinin sağlıklı yaşam biçimi davranışlarını değerlendirmek ve klinik uygulamalarında yaşam biçimi tıbbına yönelik katılımlarını incelemektir. Yöntemler: Temmuz 2018 ile Ağustos 2019 tarihleri arasında aile hekimlerinden veri toplamak amacıyla çevrimiçi bir anket geliştirildi. Tanımlayıcı ve korelasyonel bir araştırma deseni kullanıldı. Çalışmada uygulanan anket, “katılımcıların kişisel bilgileri” ve “Sağlıklı Yaşam Biçimleri Davranış Ölçeği-II”i içermektedir. Kesitsel bir çalışma olarak tasarlanan bu araştırmada, örneklem kolayda örnekleme metodu ile sekiz ülkeden belirlenmiştir, ve anketler katılımcılara elektronik ortamda uygulanmıştır. Katılımcılara anketin süresi, veri saklama işlemleri, saklama süresi, araştırmacıların kimliği ve çalışmanın amacı hakkında bilgi verilmiştir. Bulgular: Çalışmaya 131 aile hekimi katılmış olup, erkek katılımcı oranı daha yüksek bulunmuştur. Sigara içme sıklığının Türk aile hekimleri arasında anlamlı derecede yüksek olduğu tespit edilmiştir (p=0,013). Ancak, ölçek toplam puanı veya altı boyutta alınan puanlar açısından aile hekimleri arasında istatistiksel olarak anlamlı bir fark bulunmamıştır. Sonuç: Çoğu aile hekimi, bu alanda daha fazla eğitim almak istediklerini belirtmiştir, bu da tıp fakültelerinin müfredatına eğitim müdahalelerin dahil edilmesi gerekliliğini göstermektedir. Ayrıca, iyi yapılandırılmış web tabanlı kitlesel çevrimiçi kurslar, aile hekimlerinin sağlıklı yaşam biçimi davranışları ve yaşam biçimi tıbbı uygulamalarına yönelik eğitim gereksinimlerini karşılamada etkili bir çözüm olabilir.

    YouTube as a Source of Information on Inflammatory Muscle Diseases: Can It Provide Valid and Reliable Information for Patients as Well as Healthcare Professionals?

    Objective: The increasing use of the internet has resulted in its gaining a more active role in both patient and professional education, including the YouTube platform, a popular platform for sharing and watching videos. This study aimed to evaluate the quality, usefulness, and reliability of videos on inflammatory muscle diseases on YouTube, as well as their determinants for both patient and healthcare professional education. Materials and Methods: The keywords “Inflammatory muscle disease”, “Idiopathic inflammatory myositis”, “Inflammatory muscle disease treatment”, “Inflammatory myositis treatment”, “Dermatomyositis” and “Polymyositis” were searched on the site on April 22nd, 2022. According to the Global Quality scale, three categories (high quality, middle quality, and low quality) were created based on the educational value of YouTube videos. The reliability of the information was assessed using the DISCERN tool. The video parameters were compared between the quality groups to reveal the determinants of the quality. Results: Seventy-five videos of patients and 116 videos of healthcare professionals were included in the video after exclusion. The numbers of high, intermediate and low-quality videos were 27 (36%), 27 (36%), and 21 (28%) for videos for patients, and 20 (17.2%), 60 (51.8%), and 36 (31%), respectively. Only the number of likes was found to be a determinant of video quality for healthcare professionals (p<0.05), and none of the other determinants were found to predict the quality for both groups (p>0.05). Conclusion: YouTube is an open platform for information, and it includes various educational videos of various quality. Experts, such as physicians, should be encouraged to provide more content to help both patients and healthcare professionals obtain better quality information. © 2025 Elsevier B.V., All rights reserved.

    Hypermetabolic Axillary Lymph Nodes Associated With Covid-19 Vaccination in Breast Cancer Management

    A 42-year-old female patient diagnosed with invasive ductal breast ca underwent18F-fluorodeoxyglucose (FDG) positron emission tomography/ computed tomography (PET/CT) scan for staging, 1.5 cm diameter hypermetabolic lesion was observed in the lower inner quadrant of the right breast that was compatible with primary tumor [maximum standardized uptake value (SUVmax ): 10.5]. No pathological18F-FDG uptake was observed in lymph nodes whose fatty hilum was seen in the right axilla. However, in the left axilla and left deep axilla, hypermetabolic lymph nodes with a maksimum diameter of 19 mm and fatty hilum were observed (SUVmax: 8.0). In a detailed CT evaluation, these lymph nodes have thicker walls than the ones in the right axilla. The patient was questioned again and coronavirus disease-2019 (COVID-19) vaccination history (with BNT162b2, COVID-19 mRNA vaccine) was determined that was administrated to the left arm 5 days ago. Tru-cut biopsy was performed from the left aksillary lymph nodes and proved to be reactive lymphoid tissue and there was no primary or metastatic tumor in these axillary lymph node tissues. The patient was given neoadjuvant chemotherapy 4.5 months after the first18F-FDG PET/CT, and the second was performed for the treatment response evaluation. Significant regression was determined from the findings. The patient underwent right total mastechtomy. She was being followed up with adjuvant chemotherapy and radiotherapy. In conclusion, hypermetabolic lymph nodes in the axillas should be interrogated for vaccination in patients with breast cancer. Hypermetabolic lymph nodes observed on the same side of the vaccinated arm in the 18 F-FDG PET/CT scan may be related to vaccine-induced reactive lymph node enlargement. Lymph node metastasis may be excluded, especially if there are hypermetabolic lymph nodes with preserved fatty hilum in the contralateral axilla on the same side as the vaccinated arm. Active lymph nodes reactive to the vaccine become inactive after a while. © 2023 by the Turkish Society of Nuclear Medicine.

    A Novel Molecular Indicator for Inhibitor Development in Haemophilia A

    Aim: Previous studies have reported inhibitor development (ID) risk in those patients who have hemophilia A (HA) with missense mutations to be 3-10%. We investigated the association between ID risk and various features of missense mutations; including the impact directly related to amino acid group change. Materials and Methods: Missense mutations in the F8 gene, clinical findings of the patients including severity of HA, and ID status were obtained from the F8 gene variant database (http://www.factorviii-db.org/). Twenty amino acids were then classified into groups according to their side chains. All information regarding each specific mutation, as well as any impact of the mutation on the amino acid group change, was recorded. Additionally, localization (at which domain) of any changed amino acid in the F8 protein was noted. Combined Annotation Dependent Depletion (CADD), Rare Exome Variant Ensemble Learner (REVEL), Mendelian Clinically Applicable Pathogenicity and Deleterious Annotation using Neural Networks scores were applied to identify a significant cut-off value indicative of ID. Results: Three variations were identified that could be considered as useful in the prediction of ID in mild HA. The first being that among mild HA patients, 7.9% (n=70/883) with mutations causing no amino acid group changes showed ID. This rate, however, was only 2.9% in patients with mutations leading to amino acid group changes. Secondly; in patients with mutations causing no amino acid group changes affecting A2, A3 and C2 domains, ID risk was found to be higher than in patients with mutations leading to amino acid group changes. Thirdly; an association between ID and CADD and REVEL scores was observed. Conclusion: In mild HA patients, the characteristics of missense mutations in terms of amino acid group changes, and CADD and REVEL scores could be of considerable utility in the prediction of ID risk. © Copyright 2021 by Ege University Faculty of Medicine, Department of Pediatrics and Ege Children’s Foundation
