Diğer hastalardan birinde over kanseri, diğerinde nöroendokrin tümör tanısı malignite tarama tetkikleri neticesinde saptandı. Tüm hastaların kraniyal manyetik rezonans görüntüleme (MRG) incelemesinde serebellar atrofi saptandı. İki hastada anti-Yo, diğerinde ise glutamik asit dekarboksilaz antikoru (anti-GAD) pozitif saptandı. İntravenöz metilprednizolon (IVMP), intravenöz immünglobülin (IVIG) ve plazmaferez (PE) tedavileri uygulandı ancak nörolojik bulgularda anlamlı düzelme elde edilemedi. Sonuç: Serebellar sendrom semptomları ile başvuran hastalarda paraneoplastik sendromlar ayırıcı tanılarda akılda tutulmalıdır. Bu yazıda nadir görülen paraneoplastik serebellar dejenerasyon tanısı konulan üç olgu sunulmuştur.Review Citation - WoS: 11Citation - Scopus: 11One Patient, Three Providers: a Multidisciplinary Approach To Managing Common Neuropsychiatric Cases(MDPI, 2023) Özge, Aynur; Domaç, Füsun Mayda; Tekin, Nil; Aydın Sünbül, Esra; Öksüz, Nevra; Çimen Atalar, Arife; Çallı, Sümeyye Yasemin; Yılmaz Küsbeci, ÖzgeBackground: Neuropsychiatric cases require a multidisciplinary approach for effective management. This paper presented case-based discussions on migraine, dementia, epilepsy, mood disorders, neuralgia, and psychosis from the perspectives of a family physician, neurologist, and psychiatrist. The goal was to highlight the importance of collaboration between healthcare providers in managing these complex cases. Methods: The paper was based on the proceedings of the Mediterranean Neuropsychiatry Symposium, where experts from family medicine, neurology, and psychiatry came together for comprehensive case-based discussions. The CARE framework (Case Report, Appraisal, Research, and Education) was developed to guide reporting and evaluation of case reports in clinical practice. Results: Six cases were presented and discussed, highlighting the importance of a multidisciplinary approach in managing neuropsychiatric cases. The cases included chronic migraine with medication overuse, memory dysfunction with language and behavioral problems, refractory epileptic seizures with subjective sensory symptoms, bipolar affective disorder with normal pressure hydrocephalus, postherpetic neuralgia in a case with bipolar affective disorder, and psychosis with recurrent attacks with the abuse of several substances. Conclusion: A biopsychosocial multidisciplinary approach is essential for managing neuropsychiatric cases effectively on behalf of the patients and public health of the country. The CARE framework can guide the reporting and evaluation of case reports in clinical practice, ensuring that patients receive comprehensive and effective care. Healthcare providers should collaborate to provide the best possible care for patients with complex and multifaceted needs.Article Citation - WoS: 22Citation - Scopus: 20Clarifying the Relationship Between Sarcopenia and Depression in Geriatric Outpatients(Taylor & Francis Ltd, 2021) Delibas, Dursun Hakan; Eskut, Neslihan; Ilhan, Birkan; Erdogan, Esin; Karti, Dilek Top; Kusbeci, Ozge Yilmaz; Bahat, GulistanObjective We investigate the relationship between sarcopenia components and depression in geriatric outpatients, considering the effects of potential confounding factors. Methods Adults >= 60 years of age were selected from outpatient clinics. Muscle strength was assessed using handgrip strength (HGS) measured using a hydraulic hand dynamometer and chair stand test (CSST). Physical performance was evaluated by usual gait speed (UGS), nutritional status, and frailty were screened by mini-nutritional assessment (MNA) questionnaire and FRAIL scale. Depression was diagnosed through a psychiatric interview and the administration of the Geriatric Depression Scale (GDS). Results Participants with depression were similar to participants without depression regarding age (p = .055), education (p = .095), frailty (p = .857), and HGS scores (p = .053). The group with depression had longer CSST duration (p = .023), slower UGS (p = .027), and more malnutrition (p = .001). Multivariate regression analysis revealed that only the malnutrition was independently associated factor with depression after adjusting for confounding factors. Conclusions Depression is associated with malnutrition and some components of sarcopenia in geriatric outpatients. Our results revealed that sarcopenia might be associated with depression through malnutrition. If malnutrition lasts for a long time, sarcopenia may become evident in the later stages of depression.Article Evaluation of Time in Therapeutic Range in Patients With Cerebrovascular Disease Receiving Treatment With Warfarin(2021) Tamer, Pınar; İnci, İpek; Yılmaz Küsbeci, Özge; Ataç, Ceyla; Eşkut, NeslihanObjective: The efficacy of warfarin therapy correlates with the Time in Therapeutic Range (TTR). This study aimed to investigate the efficacy and target achievement rates in patients using warfarin who were followed up in stroke outpatient clinics. Methods: In this study, 103 patients who had been under warfarin treatment were retrospectively evaluated. Duration and indications for warfarin use, thrombotic or hemorrhagic complications were recorded. Target INR value was 3.0-3.5 in patients with mechanical valves and 2.0-3.0 in those using warfarin for other indications. Results: In this study, 103 patients with a mean age of 67.14±14.19, 47 men and 56 women, were included. Indications for warfarin use, respectively, were non-valvular atrial fibrillation (AF) in 61.6% of the patients, akinetic/hypokinetic segment on echocardiography (ECHO) in 12.62%, thrombophilia in 8.74%, metallic prosthetic valve in 3.88%, frequent transient ischemic attacks in 6.79%, a hypokinetic segment on ECHO with atrial fibrillation in 5.82% and vertebral artery dissection in 0.87%. 46.8% of the patients were determined to be in good control, with a mean TTR of 64.09%; also, TTR was 100% in 36.8% of them. Minor/major hemorrhagic event was noted in 10 patients (9.70%), two of whom had an intracerebral hemorrhage. Conclusion: The mean TTR value detected in this study is considerably higher than the rate reported in our country. Findings of this study suggest that close monitoring is paramount of importance and vital in patients with stroke. Regular monitoring of stroke patients using warfarin will be beneficial in terms of informing patients, providing optimal anticoagulation and preventing complications.
