Article Koroner Arter Bypass Greft Ameliyatı Geçiren Hastaların Taburculuk Sonrası Tele-Sağlık İzlemi ile Öz Etkililik ve Memnuniyet Durumunun Değerlendirilmesi(2024) Dığrak, Ebru; Ogce, Filiz; Savsar, Adile; Aygüler, MügeAmaç: Bu çalışma koroner arter bypass ameliyatı (KABG) geçiren hastaların taburculuk sonrası tele-sağlık izlemi ile öz etkililik ve memnuniyet durumunun değerlendirilmesi amacıyla yapıldı. Yöntem: Tanımlayıcı ve kesitsel tipte yapılan bu araştırmanın örneklemini bir üniversite hastanesinin kalp ve damar cerrahisi kliniğinde Temmuz 2022-Nisan 2023 tarihleri arasında KABG ameliyatı geçiren 102 hasta oluşturdu. Verilerin toplanmasında kişisel bilgi formu, 15. gün ve 30. gün hasta kontrol formu ile Barnason Etkililik Beklenti Ölçeği (BEBÖ)’nin Kardiyak Cerrahi Versiyonu kullanıldı. Bulgular: Hastaların yaş ortalaması 63,97 ± 9,75 ve %75,7’si erkekti. Hastaların %95,1’inin yanında bakmakla yükümlü olduğu bir kişi olduğu, %96,1’inin ziyaretçi kısıtlaması yaptığı, %85,4’ü diyet önerilerine uyduğu, %85,4’ü iç ve dış mekan yürüyüşleri yaptığı, %97,1’i evde ilaçlarını uygun şekilde aldığı, %99’unun dikişleri belirtilen zamanda aldırdığı ve tamamının tele-sağlık hizmeti tarafından sağlanan aramalardan memnun olduğu bulundu. Hastalarda KABG ameliyatı sonrası sırt üstü uyuma, göğüs korsesini kullanma, yataktan kalmadan önce varis çorabını giyme ve triflo topunu kullanımı 15. günden 30. güne geldiğinde anlamlı olarak azaldığı bulundu (P < 0,05). Çalışmaya alınan hastaların öz etkililik düzeylerinin orta düzeyde olduğu ve hastaların tamamının tele sağlık hizmeti ile aranmaktan memnun olduğu belirlendi. Sonuç: Elde edilen veriler sonucunda KABG ameliyatı geçiren hastaların taburculuk sonrası öz etkililiğin yetersiz olduğu ve tele-sağlık ile izlemden memnun oldukları tespit edildi. Bu nedenle hemşirelerin taburculuk sonrası tele sağlık izlem sayısının artırılarak, hastaların öz etkililiklerini geliştiren bilgilere ağırlık verilmesi önerilebilir.Article Citation - WoS: 6Citation - Scopus: 7Factors Affecting Covid-19 Fear and Burnout in Surgical Nurses(Springer London Ltd, 2023) Savsar, Adile; Karayurt, OzgulBackgroundSurgical nurses face the risk of psychological problems while trying to cope with the challenges arising from the COVID-19 pandemic.AimThis study aimed to determine levels of COVID-19-related fear and burnout and affecting factors in surgical nurses.DesignThe study has a descriptive, cross-sectional design.MethodsThe study sample included 321 nurses working in surgical units and operation rooms in Turkey. Data were gathered with a sociodemographic and occupational characteristics form, the Fear of COVID-19 Scale and the COVID-19 Burnout Scale through a Google form between 1 August and 15 October in 2021. Obtained data were analyzed with independent groups t-test, One-Way ANOVA and simple and multiple linear regression analyses.ResultsThe nurses had moderate levels of fear (20.00 +/- 6.77; Min-Max: 7-35) and burnout (29.52 +/- 10.03; Min-Max:10-50) due to COVID-19. The female gender and belief in health staff shortage were predictive of fear and burnout related to COVID-19. Age was not predictive of COVID-19 fear and receiving education about COVID-19, exposure to violence, having adequate supplies of goggles/face shields and having a limited number of aprons/work wear were not predictive of COVID-19-related burnout. Fear of COVID-19 was predictive of COVID-19 burnout.ConclusionsFemale nurses and nurses believing in health staff shortage had higher levels of fear and burnout due to COVID-19. As COVID-19 fear increased, so did COVID-19 burnout. Nurses working in surgical units should be provided with education about coping strategies taking account of the factors affecting COVID-19-related fear and burnout.Article Citation - WoS: 1Citation - Scopus: 1Investigation of Sleep Quality and Intolerance of Uncertainty in Liver Transplant Recipients: a Cross-Sectional Study(Wiley, 2025) Savsar, Adile; Karayurt, Ozgul; Kilic, Murat; Kul, Guelay AksuLiver transplant recipients experience many uncertainties after transplantation. Also, sleep problems are common among them. This study aimed to examine intolerance of uncertainty and sleep quality in liver transplant recipients. A descriptive and cross-sectional study was conducted with 117 liver transplant recipients followed in the outpatient clinic of a liver transplant center. Data were collected using a sociodemographic and clinical features form, the Intolerance of Uncertainty Scale-12 and the Pittsburgh Sleep Quality Index between May and September in 2023. The data were analyzed using descriptive statistics, correlation analysis and regression analysis. The recipients had a high mean score of 41.04 +/- 13.11 (min-max: 12-60) on the Intolerance of Uncertainty Scale-12. Age was predictive of uncertainty. Indeed, young recipients had higher intolerance of uncertainty. The recipients had a low score of 7.33 +/- 3.57 (min-max: 1-21) on the Pittsburgh Sleep Quality Index. The prevalence of sleep problems was 65%. Experiencing complications after transplantation was predictive of and worsened sleep quality. The sleep quality of the recipients had no relation with intolerance of uncertainty and inhibitory anxiety, but it had a relation with prospective anxiety.Article Sexual Life, Pregnancy and Counseling After Renal Transplantation(Turk Nefroloji Diyaliz ve Transplantasyon Hemsireleri Dernegi, 2022) Karayurt, Ö.; Savsar, A.Kidney transplantation is considered the gold standard treatment for end-stage renal disease. Transplantation improves sexual health and quality of life by reducing the risk of death and increasing the chances of long-term survival. Approximately 40% of kidney transplant recipients are women and men who are able to become parents. Although kidney transplantation gives hope to kidney failure patients desiring to have children, transplantation brings some difficulties including sexual dysfunctions and unplanned pregnancies. Women receiving immunosuppressive therapy after kidney transplantation can be affected by sexual dysfunction, adverse pregnancy outcomes and breastfeeding concerns, and for men, sexual dysfunctions are common. Patient care and follow-up during the transplant process require a multidisciplinary team approach that includes a transplant surgeon, nephrologist, nephrology and transplant nurses, and organ transplant coordinator. Nephrology and transplantation nurses and other transplantation team members have important responsibilities before, during and after transplantation. To maximize the chances of graft survival and improve quality of life, nephrology and transplantation nurses need to take a holistic approach, and provide the patient with counseling on sexuality and reproduction, and also carefully manage pre-and post-operative care. © 2022, Turk Nefroloji Diyaliz ve Transplantasyon Hemsireleri Dernegi. The Neuman Systems Model (NSM) is especially suited to holistic care as it proposes an open system approach and addresses five major interacting variables. This study aimed to investigate the applicability of the NSM when caring for a patient with electrical burns. In this case study, we planned and applied nursing care for a 20-year-old man with electrical burns based on the NSM. The study was conducted between November 25, 2018, and January 10, 2019. We used the six-item Neuman Diagnostic Tool (NDT) developed by Neuman in our interviews with the patient. We found that the electrical burns had caused multiple physiological, psychological, and sociocultural problems for our patient, most of which were physical problems, including the burn injury and subsequent limb amputation. The NDT guided our data collection and aided in our ability to determine stressors and formulate appropriate nursing diagnoses. The NSM strengthened the patient’s flexible line of defense for coping individually and facilitated the identification of deficiencies in the normal and resistance lines of defense. Yöntemler: Bu tanımlayıcı kalitatif çalışma ameliyathane hemşireleri ile yapılmıştır (n = 96). Çalışma verileri tanıtıcı özellikler formu ve yarı yapılandırılmış form kullanılarak Eylül 2020-Mart 2021 tarihleri arasında toplanmıştır. Ameliyathane hemşirelerinden “COVID-19”, “maske kullanımı” ve “karantina süreci” kavramlarını neye benzettiklerini, benzetme nedeniyle birlikte yazması istenmiştir. Veriler, “içerik analizi” yöntemiyle incelenmiştir. Bulgular: Araştırmadaki üç farklı kavrama ilişkin toplam 181 metafor üretilmiştir. “COVID-19” kavramına ilişkin benzetmeler “Kontaminasyon ve mutasyon,” “Zarar verici” ve “Başetme ve uyum süreci” olarak üç kategoride toplanmıştır. “Karantina süreci” kavramına ilişkin benzetmeler “Özgürlüğün Kısıtlaması” ve “Değişim Süreci” olarak iki kategoride toplanmıştır. “Maske kullanımı” kavramına ilişkin benzetmeler ise “olumlu yönleri,” “olumsuz yönleri” ve “hem olumlu hem de olumsuz” olarak üç kategoride toplanmıştır. Nurses play a critical role in ensuring patient safety. This study aims to determine the patient safety perceptions of nurses working in operating rooms and surgical intensive care units. Methods: This descriptive and cross-sectional study was conducted with 377 nurses working in the intensive care and operating room units of five hospitals in Izmir. Data were collected between February 8 and July 25, 2023, using the "Socio-demographic and Occupational Characteristics Form" and the "Leiden Operating Room and Intensive Care Safety Scale- LOTICS". The scale consists of five sub-dimensions: Teamwork and Awareness, Resource Management and Planning, Employee Resources and Audit, Teamwork Instructions and Preparations, and Training and Access to Information. A higher score on the scale indicates a more positive perception of working conditions. The data were analyzed using independent groups t-test, Mann-Whitney U, One Way ANOVA, Kruskal-Wallis test, and multiple linear regression. Results: The total patient safety perception score for intensive care nurses (ICN) was significantly higher (106.03 +15.93; min-max: 67-151) than that of operating room nurses (ORN) (101.81+14.24; min-max: 43-132). ICN perceived the sub-dimensions of "Resource Management and Planning", "Teamwork and Awareness", and "Teamwork Instructions and Preparations" more positively than ORN. Additionally, being married, being ICN, and having received training in team collaboration predicted nurses' perceptions of patient safety. Conclusion: Intensive care nurses exhibited higher patient safety perception scores than operating room nurses. Married nurses reported higher perception scores than single nurses, and those who received training on team collaboration had better perceptions than those who did not. Therefore, their perceptions of working conditions were more positive. Interventions designed to reduce appearance anxiety that can cause psychological problems such as depression are important. This study aimed to determine the social appearance anxiety of individuals following burns and the factors affecting it and to examine the relationship between social appearance anxiety and perceived social support. Methods: This descriptive study was conducted between November 2018 and November 2019 with 106 adult patients with burns. Data were collected using a Personal Information Form, the Social Appearance Anxiety Scale, and the Multidimensional Scale of Perceived Social Support. Shapiro-Wilk test, Independent Two Samples T-Test, One Way Analysis of Variance, Duncan's test, Pearson's correlation analysis, multiple linear regression analysis, the Q-Q plot, and the Durbin-Watson statistics were used in data analysis. Results: The social appearance anxiety scores were moderate (39.38 +/- 17.71). Being single; having a high level of education; burns on the face, head, or neck; burn-related amputation; and passing the one-week after injury period were found to significantly increase social appearance anxiety (p < 0.05). Although the perceived social support scores of the individuals with burns were high (68.34 +/- 18.08), they were found to have no correlation with social appearance anxiety (p > 0.05). Conclusions: The results of this study show that social support does not affect social appearance anxiety. In this respect, we recommend that interventions such as reconstruction and professional psychological support initiatives be prioritized for individuals with burn trauma who are evaluated to have high social appearance anxiety by the appearance anxiety scales. Methods: A descriptive and cross-sectional design was used. The study was conducted between March and September 2023. Data were collected with an online questionnaire created on Google Forms, consisting of a Nurse Information Form, a Surgery-related Pressure Injury Awareness Form, and the Attitude towards Pressure Ulcer Prevention Instrument. Results: The sample of the study consisted of 258 operating room nurses working in Turkey, Croatia, and Italy. It was found that the majority of the participants (70.90 %) did not use a risk scale, had not received education on pressure injury prevention and treatment (58.10 %) but wanted to receive it (86 %). Mean attitude scores of operating room nurses by country were at an adequate level in Turkey (42.48 ± 4.30) but not at the desired level in Croatia (37.48 ± 3.44) and Italy (36.20 ± 4.02). While there was a significant positive relationship between the awareness and attitudes of operating room nurses in Turkey (p = 0.002) and Croatia (p < 0.001), no relationship was found between these variables of nurses in Italy (p = 0.109). A statistically significant difference was found between nurses' consideration of themselves sufficient and their attitudes and awareness in all three countries (p < 0.05). It was also determined that reading articles affected nurses’ awareness in all countries. Conclusions: While operating room nurses’ attitudes towards preventing pressure injuries were adequate in Turkey, it was determined that those of the nurses in Croatia and Italy were not at the desired level. Nurses should receive regular training on surgical pressure injuries to increase their awareness and to support them in implementing the recommendations of pressure injury guidelines in accordance with institutional policy. © 2024 Tissue Viability Society / Society of Tissue Viability
