    Ameliyathanede Hasta Güvenliği Kültürünün ve Güvenli Cerrahi Kontrol Listesinin Kullanımının İncelenmesi

    Giriş ve Amaç: Ameliyathanelerde hasta güvenli kültürünün sağlanmasında Amerikan Ameliyathane Hemşireler Derneği iş sürecininbasitleştirilmesi ve standardize edilmesinin yanı sıra ekip iş birliğini, hata rapor sisteminin kullanımını önermiştir. Bu çalışmanın amacıameliyathane çalışanlarında hasta güvenliği kültürünün belirlenmesi ve güvenli cerrahi kontrol listesinin kullanımının incelenmesidir.Yöntem: Tanımlayıcı ve kesitsel araştırmanın verileri, Mart 2014-Ocak 2015 tarihleri arasında bir üniversite hastanesi merkezi ameliyathanesinde"Hasta Güvenliği Kültürü Hastane Anketi" ve "Güvenli Cerrahi Kontrol Listesi Anketi" kullanılarak toplanmıştır. Örneklemi,çalışmaya katılmayı kabul eden ameliyathanede hasta ile birebir çalışan, 96 hekim, hemşire ve anestezi ve ameliyathane teknikerioluşturmuştur.Bulgular: Katılımcıların %36.5'i hemşire, %31.3'ü hekim, %32.3'ü anestezi teknikeri olup; %41.7'sinin ameliyathanede 5 yıl ve dahaaz süredir çalıştığı saptanmıştır. Ameliyathane çalışanların %81.3'ünün Güvenli Cerrahi Kontrol Listesi'ni bilmediği ancak %76'sınınkullanmak istediği belirlenmiştir. Katılımcıların %71.9'nun hiç olay raporlamadığı, %22.9'nun 1-2 olay raporladığı saptanmıştır. Ameliyathanedeçalışanların %59.4'nün ameliyathanenin hasta güvenliği düzeyini kabul edilebilir olarak değerlendirmiştir. Çalışanlarınınameliyathanede çalışma yılları ile ölçeğin, "Üniteler içerisinde ekip çalışması", "Hatalar hakkında geribildirim ve iletişim", "Hatalar hakkınacezalandırıcı olmayan tutum", "Hasta güvenliği için hastane yönetiminin tutumu" alt boyutları arasında istatiksel olarak anlamlı birfark saptanmıştır (p = 0.017, p = 0.006, p = 0.031, p = 0.015).Sonuç: Araştırma sonuçları ameliyathane hemşirelerinin Güvenli Cerrahi Kontrol Listesi'ni tanıması ve kullanmasına katkı sağlayabilir.Ayrıca hemşirelerin bu konudaki eğitimlerinin planlanmasına ve hasta güvenliği kültürünün geliştirmesine katkı sağlayabileceği öngörülmektedir.

    Anxiety, Depression, Stress, and Self-Esteem in Turkish Parents of Children With Microtia

    Objective To describe factors affecting anxiety, depression, stress, and self-esteem in parents of children with microtia. Design Cross-sectional correlational study. Participants Parents (N = 117) of children with microtia were recruited nationally through a Turkish craniofacial non-governmental organization's social media. Main Outcome Measures Demographics, parental report of microtia experiences and concerns, the Depression Anxiety Stress Scale, and the Rosenberg Self-Esteem Scale. Results The mean parental age was 35.61 & PLUSMN; 5.96 years and 74.4% were female. The mean age of their children was 6.08 & PLUSMN; 4.22 years, 69.2% were male, 93.8% had unilateral microtia, and 14.5% had ear reconstruction. Some parents had low self-esteem (24.8%) and were in the severe range for depression (30.7%), anxiety (25.6%), and stress (21.4%). Depression (M = 7.43, SD = 5.35) was associated with worry about child's future (& beta; = 0.25, P = .013), microtia information given around birth (& beta; = -0.20, P = .024), and child no ear surgery (& beta; = -0.23, P = .008), while stress (M = 8.21, SD = 5.37) was associated with worry about the child's future (& beta; = 0.28, P = .008). Parental self-esteem (M = 29.59, SD = 5.10) was related to child having ear surgery (& beta; = 0.19, P = .047) and information about microtia provided around birth (& beta; = 0.22, P = .018). There were no significant associations with Anxiety (M = 6.04, SD = 4.82) identified by multiple regression. Conclusions While most parents were in the average to moderate range for clinical concerns, a fifth to a third of participants were in the severe range for depression, anxiety, and stress and a quarter of participants for low self-esteem. Informing families about the microtia treatment process, having the child have surgery, and interventions for reducing their future concerns may provide psychological relief for them.

    Koronavirüs hastalığı 2019 (COVID-19), şiddetli akut solunum yolu sendromu koronavirüsü 2'nin (SARS-CoV-2)neden olduğu bulaşıcı bir solunum yolu hastalığıdır. Bu hastalık ilk olarak 2019 yılında Çin'in Vuhan şehrindeortaya çıkmış, ortaya çıkışından bu yana dünya çapında yayılarak 2019-2020 koronavirüs pandemisine yolaçmıştır. Hastaneler COVID-19 salgını açısından yüksek riskli ve iyi yönetilmesi gereken kurumların başındagelmektedir. Hastanelerde de en riskli alanlar yoğun bakımlar ve ameliyathanelerdir. Salgının başlamasıylabirlikte cerrahi müdahale gerekli olan tüm hastalar, olası enfekte kabul edilerek hareket edilmelidir.Ameliyathanelerde cerrahi ekip (cerrah, hemşire, anestezist ve diğer yardımcı sağlık personeli) koordineli biçimdeolası/kesin tanılı COVID-19 hastaların yönetiminde bulaş önlemlerini acilen hayata geçirmelidir.     Determining Operating Room Nurses' Knowledge and Use of Evidence-Based Recommendations on Preventing Surgical Site Infections

    Purpose: Although surgical site infections are a common problem, it is reported that 70% can be prevented by perioperative personnel compliance with evidence-based recommendations. The present study aimed to determine operating room nurses' knowledge and use of the updated evidence-based guidelines for the prevention of Surgical Site Infection. Design: This research is a descriptive and cross-sectional study. Methods: The study was carried out with 228 operating room nurses from 81 different hospitals. Data were collected using a Personal Information Form and Questionnaire for Knowing and Implementing Evidence-Based Guidelines for the Prevention of Surgical Site Infection between April and October 2019. Findings: The nurses were aware of the guidelines and suggestions for preventing surgical site infection, and the related updates (76.3%-96.6%). However, there were variations in the rate of applying the guidelines' recommendations in the institutions (40.4%-93.4%). Compliance with the guidelines was generally good in areas under nursing management, such as wearing sterile gloves (93.4%), nail length (91.2%), maintaining normothermia (88.6%), surgical brushing (81.1%) and hair removal with a clip (74.5%). However, it was lower in areas such as a bath with soap or with an antiseptic solution (40.4%), decision to remove hair (46.5%), keeping operating room doors closed (57%), operating room traffic (62.7%), and continuing prophylactic antibiotics after surgery (89.4%). Compliance should be improved in areas such as preoperative washing and minimizing staff numbers in the operating room. Conclusions: It is important to reflect the guidelines in clinical practice, but the present study shows differences between the proposed application and actual practice. In this case, a possible solution is a range of care bundle, each consisting of a few specific suggestions, that would be selected by the institutions and the perioperative team in accordance with their surgery type and patient group.     Experiences of Adolescents and Their Families in the Short-Term After Scoliosis Surgery

    Objective: The study was performed to reveal experiences of adolescents and their families in the short-term after scoliosis surgery. Methods: This study had a qualitative descriptive design, and data were collected from adolescents with scoliosis surgery (n=17) and their family members (n=9) at in-depth interviews. Obtained data were analysed with content analysis. Results: Five main themes emerged: physical complaints, unfamiliar environment (operating room and intensive care unit), emotional changes, wanting their parents to stay with them and worries about future. Both the adolescents and their families reported that the adolescents experienced not only physical and emotional problems but also worries about their future after surgery. Conclusion: Nurses should meet the adolescents and their families and allow them to express their feelings before surgery. Informing them before surgery is important in terms of relieving their anxiety and enhancing their adaptation. Creating an appropriate environment in recovery rooms and intensive care units where the adolescents can frequently see their parents may help them have better psychology. In addition, education programs that will be offered at discharge should be designed and interactive methods should be used to allow them to share their thoughts about their future.

    Factors Related To Older Patients' Fear of Falling During the First Mobilization After Total Knee Replacement and Total Hip Replacement

    The aim of this study was to determine fear of falling in the first mobilization and affecting factors in older patients. The study had a descriptive and cross-sectional design. Data were collected in Izmir, Turkey between February 2014 and March 2016. The sample included 204 older patients undergoing joint replacement surgery. Fifty-seven-point four percent and 42.6% of the patients had total hip and knee replacement respectively. 42.2% of the patients had a severe fear of falling when they were first mobilized. There was a statistically significant difference between fear of failing in the first mobilization and the mean pain severity. In addition, the difference between fear of falling and the mean anxiety level was statistically significant. Pain and anxiety are important factors contributing to fear of falling in mobilization. The results of the study can help develop multidimensional strategies for reducing fear of falling in older people after joint replacement. (C) 2017 Elsevier Inc. All rights reserved.

    Fall Behaviors and Risk Factors Among Elderly Patients With Hip Fractures

    Objective: The aim of this study was to investigate fall preventive behaviors in elderly patients who suffered hip fractures as a result of falling. Methods: This descriptive and cross-sectional study was performed at a university hospital in Izmir, Turkey between January 2014 and December 2015. Data were collected using the Fall Behaviors Scale for Old People. This study was conducted with 103 patients who had a hip fracture caused by falling. Descriptive statistics, Mann Whitney U and Kruskal-Wallis tests were used. Results: There was a significant difference between age groups in this score (KW = 6.85, p = 0.03). The patients aged 85-96 years obtained significantly higher scores for the sub-scales of protective mobility (KW = 8.71, p = 0.01) and avoidance (KW = 6.03, p = 0.04) than patients in the other age groups. There was not a significant difference in fall prevention behaviors between the elderly with a history of a repeated falls and those without a repeated fall history. Conclusion: Although elderly people with hip fractures due to falling has highly protective behavior, they have fallen. Advanced age patient has showed more protective behavior for falling.

    Investigation of Related Factors of Operating Room Nurses' Attitudes and Awareness Towards Surgery-Related Pressure Injury Prevention in Turkey, Croatia, and Italy: Surgery-Related Pressure Injury Awareness and Attitudes

    Objective: The aim of this study is to examine related factors of operating room nurses' attitudes and awareness towards surgery-related pressure injury prevention in Turkey, Croatia, and Italy. Methods: A descriptive and cross-sectional design was used. The study was conducted between March and September 2023. Data were collected with an online questionnaire created on Google Forms, consisting of a Nurse Information Form, a Surgery-related Pressure Injury Awareness Form, and the Attitude towards Pressure Ulcer Prevention Instrument. Results: The sample of the study consisted of 258 operating room nurses working in Turkey, Croatia, and Italy. It was found that the majority of the participants (70.90 %) did not use a risk scale, had not received education on pressure injury prevention and treatment (58.10 %) but wanted to receive it (86 %). Mean attitude scores of operating room nurses by country were at an adequate level in Turkey (42.48 ± 4.30) but not at the desired level in Croatia (37.48 ± 3.44) and Italy (36.20 ± 4.02). While there was a significant positive relationship between the awareness and attitudes of operating room nurses in Turkey (p = 0.002) and Croatia (p < 0.001), no relationship was found between these variables of nurses in Italy (p = 0.109). A statistically significant difference was found between nurses' consideration of themselves sufficient and their attitudes and awareness in all three countries (p < 0.05). It was also determined that reading articles affected nurses’ awareness in all countries. Conclusions: While operating room nurses’ attitudes towards preventing pressure injuries were adequate in Turkey, it was determined that those of the nurses in Croatia and Italy were not at the desired level. Nurses should receive regular training on surgical pressure injuries to increase their awareness and to support them in implementing the recommendations of pressure injury guidelines in accordance with institutional policy. © 2024 Tissue Viability Society / Society of Tissue Viability

    Operating Room Nurses’ Metaphorical Perceptions of Coronavirus Disease 2019, Quarantine, and Mask Concepts: A Qualitative Study

    Amaç: Bu çalışmanın amacı ameliyathane hemşirelerinin COVID-19, maske kullanımı ve karantina sürecine ilişkin algılarını, metaforlar aracılığıyla ortaya çıkarmaktır. Yöntemler: Bu tanımlayıcı kalitatif çalışma ameliyathane hemşireleri ile yapılmıştır (n = 96). Çalışma verileri tanıtıcı özellikler formu ve yarı yapılandırılmış form kullanılarak Eylül 2020-Mart 2021 tarihleri arasında toplanmıştır. Ameliyathane hemşirelerinden “COVID-19”, “maske kullanımı” ve “karantina süreci” kavramlarını neye benzettiklerini, benzetme nedeniyle birlikte yazması istenmiştir. Veriler, “içerik analizi” yöntemiyle incelenmiştir. Bulgular: Araştırmadaki üç farklı kavrama ilişkin toplam 181 metafor üretilmiştir. “COVID-19” kavramına ilişkin benzetmeler “Kontaminasyon ve mutasyon,” “Zarar verici” ve “Başetme ve uyum süreci” olarak üç kategoride toplanmıştır. “Karantina süreci” kavramına ilişkin benzetmeler “Özgürlüğün Kısıtlaması” ve “Değişim Süreci” olarak iki kategoride toplanmıştır. “Maske kullanımı” kavramına ilişkin benzetmeler ise “olumlu yönleri,” “olumsuz yönleri” ve “hem olumlu hem de olumsuz” olarak üç kategoride toplanmıştır. Sonuç: Bu çalışma ameliyathanelerde çalışan hemşirelerin pandemide yaşadıkları zorlukların yansıra bu sürecin olumlu yönlerine de odaklandığını göstermektedir.

    Predictors of Anxiety and Depression in Operating Room Nurses During the Covid-19 Pandemic: a Multi-Center Cross-Sectional Study

    BACKGROUND: Operating room nurses (ORNs) who directly deal with the treatment and care of COVID-19 patients during the pandemic are at risk in terms of psychological stress and other mental health problems. OBJECTIVE: The aim of the study was to compare the anxiety and depression levels of ORNs working in different countries in the COVID-19 outbreak, and to determine the predictors and the difference between countries. METHODS: This was a descriptive and cross-sectional study. Data were collected online using a google form between February 2021-February 2022. The universe of the research is nurses who are members of national Perioperative Nurses Association (USA, Türkiye, Australia, England, India, Israel) and 1127 ORNs participated the study. In multivariate analysis, independent predictors of anxiety and depression status were examined by binary logistic regression analysis. RESULTS: The rate of anxiety is 5.46 times higher in nurses in the USA, 5.07 times higher among nurses in Australia, and 4.49 times higher for nurses in England. It was determined that the rate of depression in ORNs working in Türkiye was 5.97 times higher. It was found that the rate of depression is 1.59 times higher in ORNs living away from the family after care of COVID-19 patient. CONCLUSION: Despite some differences between countries in terms of health system, patient potential, economic and the opportunities offered, generally high levels of anxiety and/or depression indicate that all ORNs need urgent support. Identifying risk factors will help recognize anxiety and depression in ORNs and take measures to protect their mental health during the pandemic. © 2024 - IOS Press. All rights reserved.
