Article Ocular Surface Disease Severity in Polycystic Ovary Syndrome: Is This Really Significant in Daily Life?(2022) Yabaoğlu, Demet; Göde, FundaOBJECTIVE: The purpose of this study was to ascertain the extent to which the ocular surface of women with polycystic ovary syndrome is impacted. STUDY DESIGN: Included in this case-control study were 23 eyes of 23 patients (Group I) with polycystic ovary syndrome and 10 eyes of 10 healthy subjects (Group II). Polycystic ovary syndrome was diagnosed when two of the following conditions were met: oligo- or anovulation, clinical and/or biochemical signs of hyperandrogenism, and ultrasonographic documentation of polycystic ovaries. Ocular surface disease grading was performed using the Ocular Surface Disease Index questionnaire, slit lamp examination, meibomian gland secretion scoring, tear film breakup time, staining scores of ocular surfaces, the Schirmer's test, and tear osmolarity assessment. RESULTS: The study observed no significant difference in age (p=0.896). Group I was found to have a significantly higher median luteinizing hormone level (p=0.027). The Ocular Surface Disease Index scores in group I were significantly higher than those in group II (p=0.031). The prevalence of anterior blepharitis was significantly greater in group I (p=0.05). Tear film breakup time was higher in group I than in group II (p=0.026). The ocular surface disease severity was found to be significantly higher in group I (p=0.03). In group I, Ocular Surface Disease Index scores were significantly positively correlated with free testosterone, while tear osmolarity was significantly positively correlated with estradiol levels but significantly negatively correlated with dehydroepiandrosterone sulfate levels (p<0.05). CONCLUSIONS: Daily activities appear to be slightly impaired in patients with polycystic ovary syndrome due to ocular surface problems. However, environmental factors (humidity, temperature, etc.) contribute to the aggravation of ocular symptoms. Gereç ve Yöntem: 2017-2019 yılları arasında İzmir Medikalpark Hastanesi Tüp Bebek Ünitesi’nde yapılan dondurma çözme siklusları arasında 5.(Grup 1) ve 6. gün (Grup 2) embriyo transferi yapılan hastaların dosyaları retrospektif olarak incelendi. Çalışmaya 40 yaş altında olan ve tek embriyo transferi yapılan kadın olgular dahil edildi. İki grubun klinik özellikleri, gebelik ve klinik gebelik sonuçları karşılaştırıldı. İstatistiksel analizde ortalamaları karşılaştımada t-test ve oranları karşılaştırmada ki-kare testleri kullanıldı. p<0.05 istatiksel anlamlı kabul edildi. Bulgular: Çalışmaya toplam 333 hasta dahil edildi. İki yüz doksan dört hastaya 5. gün (Grup 1), 39 hastaya ise 6. gün (Grup 2) dondurma çözme transferi uygulandı. Olguların genel klinik özelliklerine bakıldığında yaş (30.0?4.6; 31.4?4.3), infertilite süresi (4.0?2.8; 4.7?2.9), önceki siklus sayısı (2.8?1.7; 3.4?1.8), elde edilen oosit sayısı (11.3?4.6; 11.9?7.2) ve elde edilen embriyo sayısı (7.3?3.2; 7.0?4.7) açısından istatistiksel anlamlı bir farklılık saptanmadı. Hastaların gebelik sonuçlarına bakıldığında ise Grup 1’de %62.2 (183/294), Grup 2’de ise %33.3 (10/39) gebelik elde edildiği saptandı (p<0.05). Klinik gebelik sonuçları incelendiğinde Grup 1’de %49.2 (183/264), Grup 2’de ise %17.9 (7/39) klinik gebelik elde edildiği görüldü (p<0.05). To evaluate the knowledge level and perspectives of female cancer patients regarding fertility preservation techniques before gonadotoxic treatment. Material and Methods. This was a prospective observational survey-based study conducted between 2016 and 2020 in Izmir Economy University Medical Park Hospital. A total of 150 female cancer patients aged 18-42 years were included. The participants completed a 17-item questionnaire, developed by the research team to evaluate their knowledge and perspectives on fertility preservation techniques. Results. The mean age of the patients was 39.5 +/- 4.9 years. Only 64.7% of the patients were referred to fertility counseling by a gynecologist, while 72.6% of the patients knew of the risk of infertility after cancer treatment. There was a significant correlation between the health status and cancer stage of the patient (p=0.003). The estimated future chance of becoming pregnant spontaneously or through fertility preservation techniques was significantly higher in patients with a higher education level (p=0.041 or 0.008, respectively). Satisfaction with the counseling process was reported as high or low by 66.7% or 20% of the patients, respectively. Conclusions. The rate of referral of reproductive-age cancer patients to fertility preservation counseling is still not satisfactory. Thus, we compared the cycle outcomes of morphologically selected intracytoplasmic sperm injection (ICSI) with hyaluronan binding physiological intracytoplasmic sperm injection (PICSI) cycles.MethodsA total of 2415 ICSI and 400 PICSI procedures of 1630 patients who underwent in vitro fertilization cycles using a time-lapse monitoring system between 2014 and 2018 were analyzed retrospectively. Fertilization rate, embryo quality, clinical pregnancy rate, biochemical pregnancy rate and miscarriage rate were evaluated, differences in morphokinetic parameters and cycle outcomes were compared.ResultsIn total, 85.8 and 14.2% of the whole cohort were fertilized with standard ICSI and PICSI, respectively. The proportion of fertilized oocytes did not significantly differ between groups (74.53 +/- 1.33 vs. 72.92 +/- 2.64, p > 0.05). Similarly, the proportion of good-quality embryos according to the time-lapse parameters and the clinical pregnancy rate did not significantly differ between groups (71.93 +/- 4.21 vs. 71.33 +/- 2.64, p > 0.05 and 45.55 +/- 2.91 vs. 44.96 +/- 1.25, p > 0.05). No statistically significant differences were found between groups in clinical pregnancy rates (45.55 +/- 2.91 vs. 44.96 +/- 1.25, p > 0.05). Biochemical pregnancy rates (11.24 +/- 2.12 vs. 10.85 +/- 1.83, p > 0.05) and miscarriage rates (24.89 +/- 3.74 vs. 27.91 +/- 4.91, p > 0.05) were not significantly different between groups.ConclusionThe effects of the PICSI procedure on fertilization rate, biochemical pregnancy rate, miscarriage rate, embryo quality, and clinical pregnancy outcomes were not superior. The retrospective cohort study included couples undergoing assisted reproductive treatment in 2023-2024 for primary infertility with a history of low fertilization rates (<30%), low blastulation rates (<20%), or male factor infertility such as teratozoospermia at the Izmir University of Economics Medical Point Hospital IVF Center. Sibling oocytes from each patient were randomly assigned to piezoelectric or CaI activation. The fertilization rate, number of degenerated embryos, number of abnormally fertilized eggs, number of day 3 grade 1 embryos, and blastocyst counts were compared among the sibling oocytes. In total, 1,105 MII sibling oocytes from 76 patients were included. Piezoelectric activation was applied to 575 MII oocytes, while CaI activation was applied to 530 MII oocytes. There was no significant difference between groups according to the activation method in the fertilization rate (piezo, 0.74% +/- 0.22%; CaI, 0.68% +/- 0.26%, p = 0.12), degeneration rate (piezo, 0.04% +/- 0.13%; CaI, 0.02% +/- 0.06%, p = 0.452), day 3 grade 1 embryo rate (piezo, 0.59% +/- 0.33%; CaI, 0.54% +/- 0.35%, p = 0.34), or blastocyst rate (piezo, 0.49% +/- 0.33%; CaI, 0.43% +/- 0.34%, p = 0.28) of sibling oocytes. Both piezoelectric and CaI activation yielded comparable results in terms of oocyte activation, with similar fertilization and blastulation rates.Article Yardımcı Üreme Tekniklerinde Oosit Kriyoprezervasyonunun Günümüzdeki Yeri ve Kliniğimizde Uygulanan Oosit Dondurma Sikluslarının Değerlendirilmesi(2022) Göde, Funda; Sertkaya, U; Emirdar, Volkan; Pala, İbrahim; Karataşlı, Volkan; Okay, GülinAmaç: Çeşitli endikasyonlar ile kliniğimizde uygulanan oosit dondurma hastaları ve sonuçlarının değerlendirilmesidir. Yöntem: Bu retrospektif araştırmaya üçüncü basamak bir hastanenin tüp bebek merkezine 2015-2020 yılları arasında başvuran, gonadotoksik tedaviler öncesi fertilite prezervasyonu uygulanan; düşük over rezervi saptanan evli olmayan kadın hastalar ve azospermi nedeniyle işlem günü eşlerinden sperm elde edilemeyen veya oosit sayısının az olması nedeniyle mikroenjeksiyon için oosit havuzlaması yapılan 18-48 yaş arası kadın hastaların oosit dondurma siklusları ve sonuçları değerlendirildi. Bulgular: Farklı endikasyonlar ile başvuran 224 kadın hastanın yapılan kontrollü ovaryen stimülasyon sonrası oosit dondurma sikluslarından elde edilen verilere göre oosit canlılık oranı %92,68, çözme siklusu başına gebelik oranı %8,66, çözme siklusu başına canlı doğum oranı ise %4,66 olarak bulundu. Sonuç: Klinik uygulamalarda yerini almış olan oosit kriyoprezervasyon sikluslarında dondurma-çözme sonrası oosit sağkalım oranları vitrifikasyon ile oldukça yüksek saptanmış olup; bu oositler ile elde edilen gebelik ve canlı doğum oranları, embriyo dondurma-çözme oranlarına yakın bulunmuştur. Vitrifikasyon tekniklerindeki gelişmeler ile yüksek canlılık ve başarı oranları yakalanan oosit dondurma-çözme uygulamalarının endikasyonlar dahilinde yardımcı üreme tekniklerinde daha güçlü bir yer alacağı düşünülmektedir.
