This study aimed to investigate the efficacy and target achievement rates in patients using warfarin who were followed up in stroke outpatient clinics.Methods: In this study, 103 patients who had been under warfarin treatment were retrospectively evaluated. Duration and indications for warfarin use, thrombotic or hemorrhagic complications were recorded. Target INR value was 3.0-3.5 in patients with mechanical valves and 2.0-3.0 in those using warfarin for other indications.Results: In this study, 103 patients with a mean age of 67.14±14.19, 47 men and 56 women, were included. Indications for warfarin use, respectively, were non-valvular atrial fibrillation (AF) in 61.6% of the patients, akinetic/hypokinetic segment on echocardiography (ECHO) in 12.62%, thrombophilia in 8.74%, metallic prosthetic valve in 3.88%, frequent transient ischemic attacks in 6.79%, a hypokinetic segment on ECHO with atrial fibrillation in 5.82% and vertebral artery dissection in 0.87%. 46.8% of the patients were determined to be in good control, with a mean TTR of 64.09%; also, TTR was 100% in 36.8% of them. Minor/major hemorrhagic event was noted in 10 patients (9.70%), two of whom had an intracerebral hemorrhage.Conclusion: The mean TTR value detected in this study is considerably higher than the rate reported in our country. Findings of this study suggest that close monitoring is paramount of importance and vital in patients with stroke. Regular monitoring of stroke patients using warfarin will be beneficial in terms of informing patients, providing optimal anticoagulation and preventing complications.Article Paraneoplastik sendrom ilişkili serebellar ataksi: üç olgu sunumu(2024) Tarı, Mahmut; Aydın, Berfin Kardelen; Köşkderelioğlu, Aslı; Yılmaz Küsbeci, Özge; Ortan, Pınar; Eşkut, NeslihanGiriş: Paraneoplastik nörolojik sendromlar (PNS), kanserin ve metastazların lokal etkisi ya da kemoterapi yan etkileri ile açıklanamayan, sıklıkla otoimmün kökenli mekanizmalarla oluşan nörolojik tablolardır. Paraneoplastik serebellar dejenerasyon (PSD) PNS’lerden biri olup ilerleyici serebellar fonsiyon bozukluğu ile ortaya çıkan bir tablodur. Olgu: Bu yazıda kliniğimize subakut-kronik ilerleyici baş dönmesi, dengesizlik, yürüme güçlüğü şikayetleri ile başvuran, yapılan tetkiklerle PSD tanısı konulan üç olgu sunulmuştur. Hastalardan birinde bilinen serviks kanseri tanısı vardı. Diğer hastalardan birinde over kanseri, diğerinde nöroendokrin tümör tanısı malignite tarama tetkikleri neticesinde saptandı. Tüm hastaların kraniyal manyetik rezonans görüntüleme (MRG) incelemesinde serebellar atrofi saptandı. İki hastada anti-Yo, diğerinde ise glutamik asit dekarboksilaz antikoru (anti-GAD) pozitif saptandı. İntravenöz metilprednizolon (IVMP), intravenöz immünglobülin (IVIG) ve plazmaferez (PE) tedavileri uygulandı ancak nörolojik bulgularda anlamlı düzelme elde edilemedi. Sonuç: Serebellar sendrom semptomları ile başvuran hastalarda paraneoplastik sendromlar ayırıcı tanılarda akılda tutulmalıdır. Bu yazıda nadir görülen paraneoplastik serebellar dejenerasyon tanısı konulan üç olgu sunulmuştur.Article The Relationship Between Daily Carbonhydrate and Fat Intake a Long With Migraine(2022) Eşkut, Neslihan; Uysal, Armağan; Ataç, CeylaThe pathophysiology of migraine is not fully understood. The trigeminovascular pathway activation, cortical spreading depression, vascular dysfunction, pro-inflammatory and oxidative states, and gut microbiota are investigated. In this study, we aimed to evaluate the relationship between diet and migraine disease characteristics of our patients. This study was conducted with episodic migraine without aura (n:30) and chronic migraine patients (n:30). Demographic data, body mass indexes (BMI), migraine characteristics were recorded from headache diaries. Food consumption were noted Daily from diet diaries. Total calorie intake and carbohydrate and fat amounts were calculated with an internet-based calorie calculator program.The mean age was 40.1±7.83 years in chronic migraine group ( group 1), and 39.1±6.09 years in group 2. The mean BMI of group 1 was significantly higher (respectively; 28.3±3.14, and 24.9±3.29). The mean duration of migraine was found longer in group 1. The mean headache attack duration was 29.9±21.85 hours in group 1, and 29.6±22.4 was in episodic migraine without aura group (group 2). There was no significant difference between both groups in terms of headache attack duration. Average Daily carbohydrate intake is 178.53±44.86 grams in group 1, and 171.42±50.67 grams in group 2. The mean Daily fat intake was 58.01±13.65 grams in group 1 and 56.62±7.51 grams in group 2. No significant difference was found between the groups in terms of Daily calorie, mean Daily fat and, average carbohydrate intake. In our study, we did not find difference in food intake between groups, but BMI of the chronic migraine group were higher. The role of diet on the migraine pathophysiology is still under investigation. An integrative approach to migraine patients by reviewing their diet will help to understand the pathophysiology of migraine, increase the quality of life of patients and prevent unnecessary drug usage.
