Article Citation - WoS: 2Citation - Scopus: 2Adrenomedullin Has No Effect on Segmental Bone Defect Healing but Increases Bone Mineral Density in Rat Model(AVES, 2023) Kaymakoğlu, Mehmet; Ciftci, E.; Korkusuz, P.; Ozdemir, E.; Erden, M.E.; Turhan, E.Objective: This study aimed to investigate the effect of adrenomedullin on the healing of the segmental bone defect in a rat model. Methods: Thirty-six Wistar rats were randomly divided into 6 groups based on follow-up periods and administered a dose of adreno-medullin hormone. In each group, bilaterally, a 2-mm bone defect was created at the diaphysis of the radius. Sodium chloride solution was administered to sham groups 3 times a week for 4 and 8 weeks intraperitoneally. Adrenomedullin was administered to the study groups 3 times a week: 15 μg—4 weeks, 15 μg—8 weeks, 30 μg—4 weeks, and 30 μg—8 weeks, respectively. After euthanasia, the segmental defects were evaluated by histomorphometric [new bone area (NBA)] and microtomographic [bone volume (BV), bone surface (BS), and bone mineral density (BMD)] analyses. Results: Although the 4-and 8-week 15 μg administered study groups had higher NBA values than the other study and control groups, the histomorphometric analysis did not reveal any statistical difference between the control and study groups regarding NBA (P >.05). In microtomographic analysis, BV was higher in the 15 μg 4-week group than 30 μg 4-week group (296.9 vs. 208.5, P =.003), and BS was lower in the 30 μg 4-week group than in the 4-week control group (695.5 vs. 1334.7, P =.005), but overall, no significant difference was found between the control and study groups (P >.05). Despite these minor differences in histomorphometric and microtomographic criteria indicating new bone formation, the BMD values of the 15 μg 8-week study group showed a significant increase compared with the control group (P =.001, respectively). Conclusion: Adrenomedullin positively affected BMD at 15 μg, but this study could not show healing in the segmental defect site at different dose regimens. Further studies are needed to assess its effects on bone tissue trauma. © 2023, AVES. Vascular responses were measured in aortic segments isolated from young (2-4 month) and old (20-24 month) male Sprague-Dawley rats. In phenylephrine-contracted intact tissues, receptor-mediated and receptor-independent endothelium-derived relaxations were induced by acetylcholine (ACh) and sarcoplasmic/endoplasmic reticulum Ca2+ ATPase (SERCA) blocker cyclopiazonic acid (CPA), respectively. In addition, CPA-induced changes in intracellular calcium levels were monitored in fura-2-loaded endothelium-denuded tissues. Real-time quantitative reverse transcription polymerase chain reaction and western blot analysis were performed to determine the transient receptor potential canonical (TRPC) 4 mRNA and protein levels. Endothelial TRPC4 mRNA levels were apparently decreased in aging rats. Immunoblot analysis showed that TRPC4 protein levels significantly decreased in intact aorta from 20- to 24-month-old rats compared to that from 2- to 4-month-old rats. ACh- and CPA-induced endothelium-dependent relaxations decreased in old rat aorta without any change in direct vasodilation induced by sodium nitroprusside. Store-operated Ca2+ entry (SOCE) induced by CPA was significantly decreased, whereas sarcoplasmic reticulum Ca2+ release was unaffected in endothelium-denuded aging rat aorta. In conclusion, TRPC4 downregulation could be associated with decreased endothelium-dependent vasorelaxations. As endothelial nitric oxide synthase is activated by SOCE-induced caveolar internalization, tracking the expression levels of SERCA, ion channels, and/or associated proteins involved in SOCE would lead to the development of novel therapeutics for age-related vasospastic disorders with dysfunctional endothelium.Article Citation - WoS: 14Citation - Scopus: 12Analysis of Risk Factors Affecting Mortality in Elderly Patients Operated on for Hip Fractures: a Retrospective Comparative Study(Turkish Assoc Orthopaedics Traumatology, 2021) Biçen, Ahmet Çağdaş; Akdemir, Mehmet; Turken, Mehmet Aykut; Cekok, Kubra; Ekin, Ahmet; Turan, Ahmet CemilObjective: The aims of this study were (1) to investigate the effects of different demographic and perioperative modalities on mortality rates and (2) to compare mortality rates between different implants in elderly patients operated on for hip fractures. Methods: In this retrospective study, a total of 314 patients who were operated on for hip fractures were included study. Patients were then divided into four groups based in their implant types: long-stem cementless bipolar hemiarthroplasty (n = 124; 102 female, 22 male; mean age = 84.2 +/- 6.4 years), standard-stem cementless bipolar hemiarthroplasty (n = 74; 48 female, 26 male; mean age = 83.5 +/- 6.9 years), antegrade intertrochanteric nail (n = 61; 35 female, 26 male; mean age = 78.5 +/- 6.8 years), and total hip arthroplasty (n = 55; 34 female, 21 male; mean age = 72.5 +/- 4.3 years). Data including gender, age, duration from injury to surgery, American Society of Anesthesiologists (ASA) score, comorbidities, use of antiplatelet agents, Barthel Index of Activities of Daily Living, type of anesthesia, operation time, preoperative hemoglobin values, blood transfusions given, duration of hospital stay, complications, and type of fracture were recorded. Results: Overall, the mean follow-up was 36.5 (range = 0 - 107) months. The overall mortality rate was 53.2%. The median survival duration was 44.2 +/- 5 months (range = 34.3 - 54). Survival rates were found significantly different among the groups (P = 0.001). In the first three years postoperatively, the mortality rate was higher in the standard-stem bipolar hemiarthroplasty group, but in the long-term follow-up, the long-stem bipolar hemiarthroplasty group exhibited the higher mortality rates. It was observed that some parameters had statistically significant effects on the mortality rates. Male gender, higher age, lower hemoglobin values, increased number of blood transfusions, ASA scores >= 3, the existence of >= 3 comorbidities were found as main predictors of increased mortality rates. Conclusion: The results of this study have shown that age, gender, preoperative hemoglobin levels, ASA scores, and comorbidities are significant factors affecting mortality in elderly patients operated on for hip fractures. Long-stem cementless bipolar hemiarthroplasty appears to show similar rates of mortality with standard-stem cementless bipolar hemiarthroplasty.Article Citation - WoS: 2Citation - Scopus: 2Analysis of the Quadratus Lumborum Muscle Activity on Leg Length Discrepancy: a Randomized Controlled Trial(Ios Press, 2022) Eryigit, Sevim; Al Kafee, Abdullah; Akan, AydinBACKGROUND: Quadratus lumborum (QL) discrete region extensions might change depending on whether leg length discrepancy (LLD) individually has any extra erector spinae action in the lumbar spine, which can result in serious injury to the lower extremities and lumbar vertebrae. OBJECTIVE: This study aims to investigate the effect of QL muscle activity on LLD by using electromyography (EMG) signals. METHODS: The study employed a randomized controlled design. A total of 100 right-handed volunteers were included in this study. All participants were assessed manually by tape measurement for LLD. EMG signals were recorded during the resting and maximal isometric contraction positions to determine QL muscle activity. The power spectral density (PSD) methods were applied to compute EMG signals. RESULTS: In maximal isometric contraction position, comparing the short right LLD (Right side = 0.00064 +/- 0.00001, Left side = 0.00033 +/- 0.0006) and short left LLD (Right side = 0.00001 +/- 0.00008, Left side = 0.00017 +/- 0.0001), it was found that the short right LLD group had significantly increased PSD of EMG values. In resting position, the short right LLD (Right side = 0.0002 +/- 0.0073, Left side = -0.00016 +/- 0.0065) had significantly increased PSD of EMG compared to the short left LLD (Right side = 0.00004 +/- 0.0003, Left side = 0.0001 +/- 0.0008) values of the QL muscle activity. The results of both groups were also statistically significant (p < 0.05). Objective: As several vaccines are still being developed and 2 have been approved, it is particularly important to perform evolutionary surveillance to identify mutations potentially affecting vaccine efficacy. Methods: DynaMut server has been used to evaluate the impact of the mutation found on SARS-CoV-2 isolates available on GISAID. Results: In this article, we analyze whole genomes sequenced from Italian patients, and we report the characterization of 3 mutations, one of which presents in the spike protein. Conclusion: The mutations analyzed in this article can be useful to evaluate the evolution of SARS-CoV-2.Article Assessing Sleep Disorders in Neuromyelitis Optica Spectrum Disorder: A Comparison of Qualitative and Quantitative Approaches(Elsevier Sci Ltd, 2025) Ture, Sabiha; Beckmann, Yesim; Ozlem, Enise Nur; Uzunkopru, Cihat; Ozakbas, SerkanObjectives: This study aimed to: 1) evaluate the macrostructure of sleep and identify the presence of sleep disorders such as hypersomnia, fatigue, apnea risk, and restless legs syndrome (RLS) in neuromyelitis optica spectrum disorder (NMOSD) patients, using both questionnaires and quantitative tests; 2) assess the correlation between the data from these questionnaires and quantitative tests in NMOSD patients, and predict which patients may require further investigation. Methods: The study population comprised 26 consecutive NMOSD patients, along with a control group of 20 healthy volunteers. Polysomnography (PSG) and the Multiple Sleep Latency Test (MSLT) were conducted, along with the administration of various questionnaires, including the Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepinesss Scale (ESS), STOP-Bang scale, RLS diagnostic questionnaire, RLS Severity Scale, and the Beck Depression Inventory (BDI). These assessments were performed prospectively to evaluate sleep quality, daytime sleepiness, the detection of obstructive sleep apnea syndrome (OSAS), the presence of RLS, the severity of RLS, and the presence of depressive symptoms. Results: An increase in the NREM1 ratio was observed in NMOSD patients compared to healthy controls, with higher values for total apnea hypopnea index (AHI), REM AHI, and NREM AHI, while the minimum O2 saturation was lower. However, no significant differences were observed between the groups regarding sleep efficiency, periodic legs movement (PLM) index, arousal index, sleep latency, and REM latency. According to the MSLT data analysis, we found that sleep latency was shorter, and hypersomnia/narcolepsy occurred more frequently. There was no significant difference in the STOP-Bang score between the two groups. The ESS score, PSQI score, presence of RLS, and body mass index (BDI) score were significantly higher in the patient group. Conclusion: Our findings suggest that the macrostructure of sleep is significantly impacted in NMOSD patients, with a higher prevalence of OSAS, hypersomnia/ narcolepsy, and RLS. We investigated the role of mTOR and other signaling molecules in the rat uterus during implantation. Female pregnant rats were divided into three groups: embryonic days (ED) 4.5, 5.5 and 6.5 according to vaginal smears. Immunohistochemical staining of mTORC1, mTORC2, IGF1, PI3K, pAkt1/2/3, ERK1 and pERK1/2 was performed on formalin fixed, paraffin embedded uterine tissue samples. pAkt1/2/3 and ERK1 also were analyzed using western blotting. We found that PI3K/Akt/mTOR and ERK/pERK were increased during the implantation period. Different amounts of mTORC1, mTORC2, IGF1, PI3K, pAKT1/2/3, ERK1 and pERK1/2 were expressed in luminal epithelium, decidual cells, embryoblast and trophoblast cells during implantation. We suggest that mTOR and associated signaling molecules may participate in implantation.Article The Association of Polypoid Growth Pattern With Lymph Node Involvement in Endometrioid Type Endometrial Adenocarcinoma(Wolters Kluwer Medknow Publications, 2023) Bezircioğlu, İncim; Yetimalar, M.H.; Kılıç, D.; Yiğit, S.Background: Tumor size is an independent predictor of lymph node metastasis and survival in the endometrioid type endometrial adenocarcinoma (EC). However, some of the ECs tend to grow towards the cavity in the polypoid pattern, which can reach very large sizes. In this study, we aimed to analyze the association of growing in the polypoid pattern of the tumor with the proportion of lymph node metastasis and extrauterine tumor spread. Methods: Four hundred seven patients were analyzed retrospectively. The effect of tumor size, tumor growing pattern, myometrial invasion, grade, and lymphovascular space invasion on the lymph node metastasis and extrauterine tumor spread were investigated. Statistical analysis consisted of unpaired t-tests for parametric data and Mann Whitney-U test for non-parametric data, whereas the Chi-square test for categorical variables. Logistic Regression, Cox Regression and multivariate analysis were used to estimate the risk predictors. Results: No association was found between the growing in polypoid pattern and lymph node metastasis (P > 0.05). In the analysis of endometrioid type EC patients who had myometrial invasion less than ½ as a subgroup, no association was found between the growing pattern and lymph node metastasis and extrauterine disease. Tumor size was found to be a statistically significant predictor of lymph node metastasis and extrauterine disease (P < 0.05). Conclusions: Lymphovascular space invasion, grade, and myometrial invasion are associated with a higher proportion of lymph node metastasis. The polypoid growth pattern of the tumor does not correlate with any histopathological parameters. © 2023 Copyright:Article Citation - WoS: 6Citation - Scopus: 2Association of Ratios of Monocyte/High-density Lipoprotein Cholesterol and Neutrophil/High-Density Lipoprotein Cholesterol With Atherosclerotic Plaque Type on Coronary Computed Tomography(Clinics Cardive Publ Pty Ltd, 2024) Okan, T.; Topaloglu, C.Objectives: The monocyte/high-density lipoprotein cholesterol (HDL-C) ratio (MHR) and neutrophil/HDL-C ratio (NHR) are markers for inflammation and dyslipidaemia, which are important factors in atherosclerosis. Studies have linked MHR and NHR to the prediction, severity and prognosis of coronary artery disease. However, no study has explored their connection to plaque stability, specifically its calcific or soft/mixed content. Methods: Monocyte, neutrophil and HDL-C levels were examined in 99 patients who had coronary computed tomographic angiography (CTA) between January and August 2023. They were divided into three groups: a group of 42 healthy individuals (group 0) with no coronary artery plaque and an Agatson score of 0, an unstable plaque group (group 1) with 31 patients displaying mixed and/or soft plaque on CTA, and a stable plaque group (group 2) with 26 patients showing only calcific plaque. Results: White blood cell (WBC), monocyte and neutrophil counts were significantly higher in group 1 patients compared to group 0 patients (group 0: WBC = 6.31 ± 0.97 × 103 cells/ µl, monocytes = 0.40 ± 0.09 × 103 cells/µl, neutrophils = 3.32 ± 0.81 × 103 cells/µl; and group 1: WBC = 7.61 ± 1.95 × 103 cells/µl, monocytes = 0.50 ± 0.11 × 103 cells/µl, neutrophils = 4.19 ± 1.36 103 cells/µl; p < 0.05). MHR and NHR were significantly higher in group 1 patients compared to group 0 patients (group 0: MHR = 0.0079 ± 0.0029, NHR = 0.063 ± 0.023 and group 1: MHR = 0.0102 ± 0.003, NHR = 0.085 ± 0.036, p < 0.05). Conclusion: The significant differences in MHR and NHR between the three groups were due to the differences between groups 0 and 1. MHR and NHR were significantly higher in group 1 patients, although there was no statistically significant difference between groups 1 and 2. Copyright © 2024 Clinics Cardive Publishing. It was intended to assess auditory and histopathological safety of ototopical use of mercurochrome solution in rats with induced tympanic membrane perforation. MATERIALS and METHODS: The study was conducted on 21 female Wistar-Albino rats which were randomly assigned into 3 groups. In all rats, perforation was performed at right tympanic membrane. Distortion product otoacoustic emissions (DPOAEs) measurements were performed at frequencies of 2000, 3000 and 4000 Hz (with L1/L2: 70 /70 dB at 2f1-f2 frequency; f2/f1 ratio: 1: 22) before recovery from anesthesia and signal-to-noise ratio (SNR) were recorded. Normal saline, 2% mercurochrome and gentamicin were given to group 1, 2 and 3 twice daily over a week, respectively. Rats were sacrificed after DPOAE measurements on day 14. Right temporal bone specimens were examined under light microscope after processing. RESULTS: Based on DPOAE results, there was no significant difference among groupst before treatment. On day 14, significant differences were found in DPOAE measurements at 3000 and 4000 Hz, and in mean SNR values in 2% mercurochrome and gentamicin groups when compared to normal saline group while no significant difference was detected at 2000 Hz among groups. In addition, significant degeneration was detected in Corti organs, spiral ganglions and stria vascularis in groups 2 and 3. CONCLUSION: In this study, it was observed that mercurochrome use in external otitis and otitis media with tympanic membrane perforation could cause ototoxicity and concluded that the solution should be used cautiously.Article Autophagy-Related Genes Affect the Survival of Multiple Myeloma Patients Depending on Chromosomal Abnormality(Walter De Gruyter Gmbh, 2022) Ayna Duran, Gizem; Benderli Cihan, YaseminBackground: Targeting autophagy at gene level may be promising in multiple myeloma (MM) treatment depending on chromosomal abnormality (ABN) status.Objectives: We aimed to investigate the role of ABN on survival of MM patients and to identify prognosis related autophagy-related genes (ARGs) for patients with or without ABN.Methods: Gene intensity values of 222 ARG for 548 MM patients were obtained from the Affymetrix Human Genome U133 Plus 2.0 Array (GPL570) platform containing 54,675 probes (GSE24080). A dataset containing data from 1576 MM patients with 1q21 amplification (GSE4204, GSE4452, GSE4581, and GSE2658) was used for validation. Survival analysis of the patients was analyzed using univariate and multivariate Cox regression method with the help of R3.53 programming language and Kaplan-Meier graphics were created. The Gene Ontology enRIchmentanaLysis and visuaLizAtion (GOrilla) tool was used to define the related biological processes and pathways.Results: The overall survival (OS) and event-free survival (EFS) in all MM patients were strongly influenced by ABN. In the group of patients with ABN, 41 ARGs were found to be important in prognosis, whereas in the group of patients without ABN, 13 ARGs were found to be important in prognosis. CDKN1A, FKBP1B, FOXO3, and NCKAP1 ARGs were commonly significant in both groups and found to be survival triggering.Conclusions: The classification of MM patients according to the absence or presence of ABN is important in the determination of survival status. Detection of survival related ARGs in patients with chromosomal anomalies may be a new therapeutic target in treatment.Article Bioinformatics Based Drug Repurposing Approach for Breast and Gynecological Cancers: RECQL4/FAM13C Genes Address Common Hub Genes and Drugs(Galenos Publ House, 2025) Duran, Gizem AynaObjective: The prevalence of breast cancer and gynaecological cancers is high, and these cancer types can occur consecutively as secondary cancers. The aim of our study is to determine the genes commonly expressed in these cancers and to identify the common hub genes and drug components. Materials and Methods: Gene intensity values of breast cancer, gynaecological cancers such as cervical, ovarian and endometrial cancers were used from the Gene Expression Omnibus database Affymetrix Human Genome U133 Plus 2.0 Array project. Using the linear modelling method included in the R LIMMA package, genes that differ between healthy individuals and cancer patients were identified. Hub genes were determined using cytoHubba in Cytoscape program. “ShinyGo 0.80” tool was used to determine the disease-specific biological KEGG pathways. Drug.MATADOR from the ShinyGo 0.80 tool was used to predict drug-target relationships. Results: The RecQ Like Helicase 4 and Family with Sequence Similarity 13 Member C genes were found to be similarly expressed in breast cancer and gynaecological cancers. Upon KEGG pathway analyses with hub genes, Drug.MATADOR analysis with hub genes related to cancer related pathways was performed. We have determined these gene/drug interactions: NBN (targeted by Hydroxyurea), EP300 (targeted by Acetylcarnitine) and MAPK14 (targeted by Salicylate and Dibutyryl cyclic AMP). Conclusion: The drugs associated with hub genes determined in our study are not routinely used in cancer treatment. Our study offers the opportunity to identify the target genes of drugs used in breast and gynaecological cancers with the drug repurposing approach.Conference Object Citation - WoS: 3Citation - Scopus: 4Birth Outcomes After Inadvertent Use of Category X Drugs Contraindicated in Pregnancy: Where Is the Real Risk?(Turkish J Pediatrics, 2018) Ozturk, Zeynep; Olmez, Ercument; Gurpinar, Tugba; Vural, KamilDrugs contraindicated in pregnancy are medicines that should be avoided by pregnant women, since they carry a concern for teratogenicity or there is no indication for their use during pregnancy. It does not mean that exposures to these drugs always cause harm. The aim of the present study was to investigate the risk of adverse outcomes following maternal exposure to the drugs contraindicated in pregnancy. We retrospectively analyzed prenatal drug exposure records of the pregnant patients referred to the clinical pharmacology consultation service in a tertiary-level university hospital from January 2007 until December 2012. Exposures to category X drugs (CXD) contraindicated in pregnancy were evaluated. After the expected date of delivery, we collected data about pregnancy complications and the outcomes. For comparison the women in the exposed group (N=52) were matched with a control group (N=162) of pregnant women without teratogenic exposure. We observed only one baby born with a birth defect (congenital cryptorchidism) in CXD group (2.6%) and four in control group (RR 0.91; 95% CI 0.10-7.94). The rates of adverse pregnancy outcomes including miscarriage, preterm birth and congenital abnormality were not significantly different from controls. However, the rate of elective termination of pregnancy was higher in women exposed to CXD while pregnant (RR 2.54; 95% CI 1.11-5.80, p = 0.027). Contraceptive failure and unintended pregnancy are the reasons for inadvertent drug exposure and choosing abortion. The high perception of teratogenic risk among pregnant women may cause terminations of pregnancies. Individual risk assessment and avoiding the phrase 'CXD' or 'contraindicated in pregnancy' in counseling may help to reduce maternal concerns about medication use in pregnancy.Article Can Video Head Impulse Testing Be Used To Estimate the Involved Canal in Benign Paroxysmal Positional Vertigo(Aves, 2024) Uslu, Mustafa; Egrilmez, Murat; Miman, Murat CemBACKGROUND: There may be confusion about which canal is involved in patients with benign paroxysmal positional vertigo (BPPV), especially with those that have subtle findings. The study aimed to determine if video head impulse testing may be used in such patients as a diagnostic tool. Symptom scoring and treatment efficiency in BPPV are essential parts of the process. Therefore, inventories like "Dizziness Handicap Inventory" may be useful in this regard. METHODS: Patients with posterior and lateral canal BPPV were included. Video head impulse testing was performed prior to treatment and 1 week after treatment. Vestibuloocular reflex (VOR) gains were noted and compared to the opposite side. The presence of correction saccades was noted as well. Also, pretreatment and posttreatment Dizziness Handicap Inventory scores were compared. RESULTS: Fifty-seven patients were diagnosed with posterior canal BPPV, and sixteen were with horizontal canal BPPV. In patients with posterior canal BPPV, there was no difference between the involved canal VOR gains and the other canals on the same side (P =.639). The involved horizontal canal did not differ from the opposite horizontal canal. Patients with lateral canal BPPV show more significant improvement after treatment compared to patients with posterior canal BPPV. Data was collected using the following forms: a socio-demographic characteristics form, the revised scale for caregiving self-efficacy (RSCSE), the dementia knowledge assessment scale (DKAS) and the revised form of the multidimensional scale of perceived social support (RFMSPSS). Data was assessed through descriptive statistics and multivariate regression analysis.FindingsThe participants' RSCSE, DKAS and RFMSPSS mean scores were 1125.89 & PLUSMN; 417.18 (range:0-1800), 15.70 & PLUSMN; 6.06 (range:0-34), and 52.72 & PLUSMN; 20.07 (range:12-74), respectively. Analysis indicated that DKAS and RFMSPSS predicted positive caregiving self-efficacy scores but it was not statistically significant for DKAS (R2:0.209, F:13.077, p < 0.001). These variables accounted for 21% of total variance of caregiving self-efficacy. RFMSPSS predicted positively and statistically significantly (& beta;:0.461, p < 0.001) the total score of the self-efficacy scale.ConclusionsThe perceived social support of caregivers of PwD is an important predictor of the self-efficacy level of caregivers.Clinical EvidenceIn order to enhance improving the quality of the caregiving process, caregivers should be strengthened, and caregiving self-efficacy should be comprehensively evaluated to enable assistance to the caregiver. The social support of caregivers is an important predictor of caregiving self-efficacy. The aim of this study was to determine the changes in distribution of HCV genotypes and risk factors. Materials and Methods: In this retrospective single-center study, HCV genotyping results of 558 patients were evaluated in between 2005 and 2016.Three different HCV genotyping assays were used during the 12-year study period;restriction fragment length polymorphism (RFLP), Abbott Real Time HCV Genotype II and Bosphore HCV genotyping kit. Results: The most prevalent HCV genotype was genotype 1 detected in 88.4% of the patients followed by genotype 3 (5.2%),genotype 4 (2.9%),genotype 2 (2.1%), mixed genotypes (1.1%) and genotype 5 (0.3%). Genotype 1a showed an increasing prevalence. There were 19 patients (3.4%) either of foreign nationalities or Turkish citizens living abroad. Genotype 3 was the most common type among these patients which 10.3% had intravenous drug use history. Syrian migrant population differed in terms of HCV genotypes. Genotype 5 detected in two Syrian patients, which is the first report of HCV type 5 in Western Turkey. Among the HCV genotype 4 infected patients, 31.3% were Syrians. Conclusion: Our study showed that although genotype 1b dominance continues, the distribution and prevalence of HCV genotypes are changing in our region mainly due to migration and increase in the frequency of patients with non-traditional risk factors such as intravenous drug use. Her mother had been diagnosed with medullary thyroid carcinoma (MTC), and treated six years ago. Heterozygous mutation of the RET proto-oncogene at c634 had been detected in her mother. Genetic analysis showed the presence of the same mutation in our patient. Thyroid functions were normal. Serum calcitonin level was found mildly elevated. Parathormone (PTH) and carcinoembrionic antigen (CEA) levels were normal. Prophylactic thyroidectomy and sampling of cervical lymph nodes were performed. Histopathologic examination revealed hyperplasia in thyroid C cells, and reactive lymphadenopathy. The risk of MTC has been reported 100% through the life of patients with RET protooncogene mutation. It has been reported that particularly patients with c634 mutation have more risk of occurence of metastatic and progressive/recurrent MTC. Prophylactic thyroidectomy, cervical lymph node dissection before 5-years-of-age should be considered for these patients.Article The Clinical Predictors of Biochemical Hyperandrogenemia and Its Relation To Treatment Resistance in Women With Acne(Termedia Publishing House Ltd, 2025) Altunel, Cemile Tugba; Tatlican, SemihIntroduction: The prevalence of biochemical hyperandrogenemia (BHA) in female acne varies across studies. While certain phenotypic features may suggest hormonal evaluation, clinical predictors of BHA are unclear. Furthermore, the predictors of treatment outcome remain inconclusive, and despite common belief, no strong evidence links BHA to treatment resistance. Aim: To identify determinants of BHA and treatment response in female acne. Material and methods: Female acne patients who underwent hormonal tests (androstenedione, DHEAS04, E2, FSH, LH, free testosterone, prolactin, SHBG, TSH, total testosterone, and 17-OHP) from January 2020 to September 2022 were analysed for associations of clinical parameters with BHA, PCOS, and treatment resistance. Results: Among 86 females (mean age: 24, range: 14-41), acne categories were as follows: persistent (46.5%), adult-onset (26.7%), recurrent (19.8%), and adolescent (7%). Clinical and BHA rates were 65.1% and 70.9%, respectively. The most common elevated hormones were 17-OHP (65%) and androstenedione (40%). Hirsutism and truncal acne were associated with BHA. High DHEAS04 and menstrual irregularity were linked to the persistent category, and 17-OHP elevation was related to a chronic course. PCOS prevalence (17.4%) was associated with high DHEAS04, Free Androgen Index, TT, low E2, and hirsutism. Persistent/recurrent acne and hirsutism were associated with treatment failure. Conclusions: The persistent course and prolonged duration of acne in females, combined with hirsutism and truncal location, are associated with BHA. Patients without androgenic signs may have BHA, and PCOS diagnosis can be established through appropriate referral. A lot of effort has been put in developing PI3K in-hibitors, opening promising avenues for the treatment of cancer. Among these, PI3K alpha specific inhibitor alpelisib was approved by FDA for breast cancer and other alpha-isoform specific inhibitors such as inavolisib and serabelisib reached clinical trials. However, the mode of action of these inhibitors on mutated PI3K alpha and how they interact with mutant structures has not been fully elucidated yet. In this study, we are revealing the calculated in-teractions and binding affinities of these inhibitors within the context of PIK3CA hotspot mutations (E542K, E545K and H1047R) by employing molecular dynamics (MD) simulations. We performed principal component analysis to understand the motions of the protein complex during our simulations and also checked the correlated motions of all amino acids. Binding affinity calculations with MM-PBSA confirmed the consistent binding of alpelisib across mutations and revealed relatively higher affinities for inavolisib towards wild-type and H1047R mutant structures in comparison to other inhibitors. On the other hand, E542K mutation significantly impaired the interaction of inavolisib and serabelisib with PI3K alpha. We also investigated the structural relationship of the natural ligand ATP with PI3K alpha, and interestingly realized a significant reduction in binding affinity for the mutants, with potentially unexpected implications on the mechanisms that render these mutations oncogenic. Moreover, correlated motions of all residues were generally higher for ATP except the H1047R mutation which exhibited a distinguishable reduction. The retrospective cohort study included couples undergoing assisted reproductive treatment in 2023-2024 for primary infertility with a history of low fertilization rates (<30%), low blastulation rates (<20%), or male factor infertility such as teratozoospermia at the Izmir University of Economics Medical Point Hospital IVF Center. Sibling oocytes from each patient were randomly assigned to piezoelectric or CaI activation. The fertilization rate, number of degenerated embryos, number of abnormally fertilized eggs, number of day 3 grade 1 embryos, and blastocyst counts were compared among the sibling oocytes. In total, 1,105 MII sibling oocytes from 76 patients were included. Piezoelectric activation was applied to 575 MII oocytes, while CaI activation was applied to 530 MII oocytes. There was no significant difference between groups according to the activation method in the fertilization rate (piezo, 0.74% +/- 0.22%; CaI, 0.68% +/- 0.26%, p = 0.12), degeneration rate (piezo, 0.04% +/- 0.13%; CaI, 0.02% +/- 0.06%, p = 0.452), day 3 grade 1 embryo rate (piezo, 0.59% +/- 0.33%; CaI, 0.54% +/- 0.35%, p = 0.34), or blastocyst rate (piezo, 0.49% +/- 0.33%; CaI, 0.43% +/- 0.34%, p = 0.28) of sibling oocytes. Both piezoelectric and CaI activation yielded comparable results in terms of oocyte activation, with similar fertilization and blastulation rates.
