    An Incidental Study of the Effects of COVID-19 Lockdowns on the Muscle Strength of Patients With Postpolio Syndrome: Do These Patients Need a Break

    Introduction: This study was designed before the emergence of the COVID 19 pandemic, to assess the annual changes in muscle strength in relatively young Turkish people with postpolio syndrome (PPS). After we completed the baseline measurements, the COVID 19 pandemic emerged and lockdowns followed right after. Thus, the purpose of the study turned to how these lockdowns, which induced a sedentary lifestyle, have affected muscle strength in participants with PPS compared to healthy controls. Materials and Methods: This study included 49 hip joints (21 PPS vs. 28 healthy controls) and 51 knee joints (23 PPS vs. 28 healthy controls). Concentric isokinetic testing of bilateral hip and knee muscles was performed in all participants with PPS and controls just before the outbreak of the COVID 19 pandemic. The measurements were repeated after 1 year of lockdowns. Results: Perceived physical activities were reported to be decreased by 35.9% and 32.2% in participants with PPS and controls, respectively. No significant changes were observed in muscle strengths, and these changes did not differ between PPS and healthy controls. Conclusion: This is the first and possibly the only study to show that the muscle strengths of participants with PPS did not change significantly during 1 year of physical restraints due to the COVID 19 pandemic. These findings may support the basic principle in the rehabilitation of participants with PPS, which includes conserving energy and avoiding the overuse of muscles. However, more data from studies with a larger number of participants and longer follow ups, which will hopefully never happen again in a situation like this, are required to draw a conclusion.

    A New Scoring System for the Evaluation of Ibrutinib-Associated Arrhythmias in Chronic Lymphocytic Leukemia: the ACEF Score

    Amaç: Kardiyak dokuda Bruton tirozin kinaz inhibisyonu, stres sırasında dokunun korunmasından sorumlu olan PI3K-AKT sinyal yolunun inhibisyonuna neden olur. Bu nedenle aritmi riskinde bir artış vardır. Bu çalışma, yaş, kreatinin ve ejeksiyon fraksiyonu komponentlerini temel alan ve basit bir puanlama sistemi olan Yaş-Kreatinin-Ejeksiyon Fraksiyonu (ACEF) skoru ile bu riskin tahmin edilebilmesini araştırmaktadır. Gereç ve Yöntemler: Kronik lenfositik lösemi (KLL) tanısı alan ve en az 1 yıldır ibrutinib tedavisi alan hastalar ekokardiyografi ve Holter elektrokardiyografi ile değerlendirildi ve sonuçlar tedavi almayan KLL hastalarından oluşan kontrol grubuyla karşılaştırıldı. ACEF skoru, yaş/sol ventriküler ejeksiyon fraksiyonu+1 (kreatinin >2,0 mg/dL ise) formülüyle hesaplandı. Bulgular: Hastaların aritmi gelişimi değerlendirildiğinde paroksismal atriyal fibrilasyon (PAF) dışındaki aritmi türleri açısından kontrol ve ibrutinib grupları arasında istatistiksel olarak anlamlı bir fark bulunmadı. PAF, ibrutinib kullanmayanlar ile kullananlar arasında %8’e karşı %22 (p=0,042) oranında meydana gelmekteydi. İbrutinib kullanan hastalarda ACEF skorunun >1,21 olması, PAF gelişimini %77 duyarlılık ve %75 özgüllükle öngörmekteydi (eğri altındaki alan: 0,830, %95 güven aralığı: 0,698-0,962, p<0,001). Sonuç: ACEF skoru, KLL tanısı alan ve ibrutinib başlanması planlanan hastalarda PAF gelişimini öngören bir risk skoru olarak kullanılabilir.

    Ocular Flutter-Myoclonus-Ataxia Syndrome After mRNA BNT162b2 COVID-19 Vaccine: A Case Report

    Ocular flutter (OF) is a rare oculomotor syndrome. The most common etiologies are paraneoplastic, postinfectious and toxic-metabolic. However post-vaccinal etiology was rarely reported in OF. Here, we reported a post-vaccinal clinical syndrome characterized by OF-myoclonus and ataxia associated with oligoclonal bands (OCBs). A 60-year-old male who presented with dizziness, unsteady gait, involuntary movements, involuntary conjugate eye oscillations and extremity jerks that started 3 days after the second dose of mRNA BNT162b2 Covid-19 vaccine. Routine biochemical and serological analysis were within normal limits. No pathological finding was detected in brain MRI. Paraneoplastic and autoimmune encephalitis tests were unremarkable in cerebrospinal fluid (CSF). Oligoclonal bands were positive in CSF. This is the first description of the relationship between vaccines against SARS-CoV-2 and the clinical syndrome of OF, Myoclonus and Ataxia (OFMAS). Humoral immune mechanisms seem to play an important role in OFMAS. Presence of OCBs in CSF may also be associated with this condition. © 2022 by Turkish Association of Neuropsychiatry.

    The Results of Environmental Enrichment and Neurodevelopmental Treatment in Shprintzen-Goldberg Syndrome: a Case Report

    Purpose: This case report aimed to investigate the efficacy of the physiotherapy program in a child with Shprintzen-Goldberg syndrome (SGS). Methods: A 9-month-old case diagnosed with SGS by a medical geneticist was the focus of the study. Congenital pes varus, craniosynostosis and craniofacial deformities, mental, social, emotional and motor retardation, regulation and sensory hyperreactivity symptoms were observed in the case. The physiotherapy program consists of environmental enrichment and Neurodevelopmental Treatment (NDT) approaches twice a week for 12 months and was conducted by a physiotherapist at Karadeniz Eregli Private Gökkuşagi Special Education and Rehabilitation Center. Gross motor function and disability level were assessed using Gross Motor Function Measurement-88 (GMFM-88) and Gross Motor Function Classification System (GMFCS), respectively. The success of physiotherapy goals was determined by Goal Attainment Scaling (GAS). Results: At 12 months follow-up, GMFM-88 score increased from 5.52% to 45.47% and GAS total scores increased from -6 to +6 point. Conclusions: The literature review shows that no previous study assessed the effectiveness of 12 months physiotherapy program in a child with SGS. We consider that physiotherapy including targeted NDT approaches can support motor development in rare cases, such as SGS characterized by severe motor involvement. © 2023 Turkish Physiotherapy Association.
