    Long-Stem Prosthesis for Intertrochanteric and Femoral Neck Fractures in the Elderly: Retrospective Comparison of Short-Term Clinical Outcomes

    Objective: Hip fracture treatment in elderly patients remains challenging. This study aimed to evaluate short-term clinical results of long-stem cementless arthroplasty in elderly patients with femoral neck and intertrochanteric fractures. Materials and Methods: We collected data of patients we treated with long-stem cementless arthroplasty between January 2011 and December 2019 due to hip fractures. This study included 48 patients (>= 65 years of age) with femoral neck and intertrochanteric fractures. The patients underwent either bipolar or total hip arthroplasty. A 2-year follow-up was conducted in all the patients. Outcomes were evaluated with the Harris hip score (HHS), Koval categories, complications and hip and mid-thigh pain. We compared clinical results of patients with femoral neck fractures and those with intertrochanteric fractures. Results: The mean follow-up period was 57 (34-92) months. Sixteen patients had femoral neck fractures and 32 had intertrochanteric fractures. No difference in demographic characteristics and comorbidities was noted between groups. The mean preoperative Barthel index of activities of daily living score was 79. At the last follow-up, the mean HHS of the control group was 77 and the mean Koval categories score was 3. Revision surgeries were conducted in two patients due to dislocation. Infection was observed in one patient and was treated by debridement and anti-biotherapy. Implant loosening and periprosthetic fractures were not noted. Conclusion: With fair outcomes, long-stem cementless arthroplasty is a potential treatment for hip fractures in elderly patients. A longer follow-up to evaluate the efficacy of these prostheses is warranted.

    A Preliminary Study of Possible Fibrotic Role of Meprin Metalloproteases in Scleroderma Patients

    Objectives: This study aims to investigate the possible fibrotic role of meprin metalloproteases and possible fibrotic effects of activator protein-1 (AP-1) in scleroderma patients. Patients and methods: Between April 2018 and April 2019, a total of 85 scleroderma patients (9 males, 76 females; mean age: 54.9 +/- 12.1 years; range, 22 to 80 years) who met the 2013 American College of Rheumatology/European League Against Rheumatism criteria and 80 healthy control individuals (10 males, 70 females; mean age 42.9 +/- 10.2 years; range, 19 to 65 years) were included. Patients' data and blood samples were collected. Messenger ribonucleic acid expressions of interleukin (IL)-6, AP-1 subunits, and tumor necrosis factor-alpha (TNF-alpha) were analyzed by quantitative real-time polymerase chain reaction. Serum meprin alpha and beta protein levels were analyzed using the enzyme-linked immunosorbent assay. Results: Meprin alpha and meprin beta protein levels increased in scleroderma patients. The AP-1 subunits (c-Fos, c-Jun), IL-6, and TNF-alpha increased in scleroderma patients, compared to controls. Conclusion: Our results provide evidence showing that increased meprins levels may be related to AP-1 levels and increased meprins levels may responsible for increased inflammatory TNF-alpha and IL-6 levels. All these data suggest meprins as promising therapeutic targets to restore the balance between inflammation and extracellular matrix deposition in scleroderma.

    Virtual Drug Screening Study to Discover Novel ERAP1 Allosteric Site Inhibitors for the Treatment of Ankylosing Spondylitis (AS)

    Endoplasmic reticulum aminopeptidase 1 (ERAP1) is one of the key molecules in the antigen presentation process. To date, associations of ERAP1 with Ankylosing Spondylitis (AS) have been revealed with strong data. As such, to target the allosteric site of ERAP1 exhibits a therapeutic potential in the treatment of AS. In this paper, 9,800 ligands from “FDA-Approved Drugs'', “World-not-FDA Approved Drugs'', and “Drugs in Clinical Trials'' datasets of ZINC15 database were screened to the allosteric site of ERAP1. The best scored drugs are filtered with ADME analysis, the toxicity and bioactivity profiles of the discovered drugs and the known inhibitors were investigated. Results revealed that ZINC000100052688 (Ventavis), ZINC000004217466, and ZINC000024760115 (Dactolicib) follow the Lipinski’s rule of five and have -10.0 kcal/mole, -9.8 kcal/mole, and -9.7 kcal/mole binding affinities to allosteric site of ERAP1, respectively. Furthermore, ZINC000004217466 is the most promising since it has high protease and enzyme inhibitory activity with no toxicity. Due to that to date, only few chemical ligands recognizing ERAP1 regulatory site have been synthesized, to reveal possible repurposable drugs is quite promising, and ZINC000004217466 is the best candidate among 9,800 drugs since it has rather binding affinity, proper chemical properties, no toxicity, and high bioactivity in the inhibition of ERAP1 regulatory site.

    25-Hydroxyvitamin Levels in Sjögren’s Syndrome: Is It the Right Time to Dismiss the Case or Not

    Objectives: This study aimed to investigate whether patients with primary Sjögren syndrome (SjS) have different levels of 25 OH-D3 (vitamin D) when compared to healthy populations and whether differences in 25 OH-D3 correlated with disease activity or markers. Methods: Eighty-eight female patients with SjS and 3,338 age-matched healthy female controls were included in this study. 25 OH-D3 levels were compared with healthy controls. Then the patients were stratified according to their 25 OH-D3 levels, either insufficient/deficient or normal (<50 nmol/L or ≥50 nmol/L). The disease activity was evaluated using The EULAR SjS disease activity index (ESSDAI) and its components. Correlation analyses were also performed for a possible correlation with disease characteristics and markers of activity. Results: No differences in 25 OH-D3 levels were found between SjS and healthy populations (p>0.05). No correla- tions were found between patient characteristics or labo- ratory values (p>0.05). Conclusions: This study did not find a link between disease characteristics and disease activity and 25 OH-D3 levels. Prospective studies with more patients should be conducted to reach a conclusion.

    YouTube as a Source of Information on Inflammatory Muscle Diseases: Can It Provide Valid and Reliable Information for Patients as Well as Healthcare Professionals?

    Objective: The increasing use of the internet has resulted in its gaining a more active role in both patient and professional education, including the YouTube platform, a popular platform for sharing and watching videos. This study aimed to evaluate the quality, usefulness, and reliability of videos on inflammatory muscle diseases on YouTube, as well as their determinants for both patient and healthcare professional education. Materials and Methods: The keywords “Inflammatory muscle disease”, “Idiopathic inflammatory myositis”, “Inflammatory muscle disease treatment”, “Inflammatory myositis treatment”, “Dermatomyositis” and “Polymyositis” were searched on the site on April 22nd, 2022. According to the Global Quality scale, three categories (high quality, middle quality, and low quality) were created based on the educational value of YouTube videos. The reliability of the information was assessed using the DISCERN tool. The video parameters were compared between the quality groups to reveal the determinants of the quality. Results: Seventy-five videos of patients and 116 videos of healthcare professionals were included in the video after exclusion. The numbers of high, intermediate and low-quality videos were 27 (36%), 27 (36%), and 21 (28%) for videos for patients, and 20 (17.2%), 60 (51.8%), and 36 (31%), respectively. Only the number of likes was found to be a determinant of video quality for healthcare professionals (p<0.05), and none of the other determinants were found to predict the quality for both groups (p>0.05). Conclusion: YouTube is an open platform for information, and it includes various educational videos of various quality. Experts, such as physicians, should be encouraged to provide more content to help both patients and healthcare professionals obtain better quality information. © 2025 Elsevier B.V., All rights reserved.

    Unresolved Pain in a Middle-Aged Woman: an Integrated Family Medicine Approach

    47 years old woman had applied to the outpatient clinic as she was suspicious because of COVID-19. The patient has non-specific symptoms such as fever, pain, and anosmia. The family physician has prescribed Enfluvir, vitamin D, etol-fort tb . After the medication, she has come to the clinic again because of the not finished pain. The family physician that the first one consulted has ordered Magnetic Resonance Imagining for the sacroiliac region as the patient told that the pain when she was sitting for at least two and half years and nowadays her pain was more although she was taking her painkillers. According to the non-contrast, MRI of the sacroiliac joint: “ joint surfaces exhibit irregularity and narrowing. In the posterior, more prominent articular surfaces on the right in the fat-suppressed axial and coronary sequences, there is focal bone marrow edema and sacroiliitis activation. The right adnexa within the sections showed an 8 cm cystic appearance. MRI and USG of the lower abdomen are advised.”. The lab and also with IV contrast, MRI of the pelvis, and ultrasonography were planned with the rheumatology and gynecology consultations.

    Matrix Metalloproteinase-2 and -3 Levels in Patients With Behçet's Disease and Implication for the Presence of Vascular Aneurysm or Neurologic Involvement

    Background: Behçet's disease is a systemic vasculitis affecting both arteries and veins, as well as causing recurrent inflammatory multiorgan disease. Vascular involvement is associated with increased mortality and morbidity. Matrix metalloproteinases are released at sites of inflammation and degrade various components of the extracellular matrix. Increased levels of metalloproteinase-9 and metalloproteinase- 2 have been previously reported in Behçet's disease. Methods: In this cross-sectional study, metalloproteinase-2 and metalloproteinase-3 serum levels were investigated in 103 patients with Behçet's disease and 69 healthy controls, using Invitrogen immunoassay human metalloproteinase-2 and metalloproteinase-3 ELISA kits. Results: Serum metalloproteinase-2 and metalloproteinase-3 levels were significantly higher in the Behçet's disease group compared to healthy controls. Besides, serum metalloproteinase-3 levels were significantly higher in subgroups of Behçet's disease with aneurysmal vascular involvement and with neurological involvement. However, metalloproteinase-2 and metalloproteinase-3 serum levels did not show a positive correlation with disease activity. Conclusion: Metalloproteinase-2 and -3 may contribute to the complex pathogenesis of Behçet's disease. More importantly, the detection of very high serum levels of metalloproteinase-3 may predict the formation of an aneurysm, or possibly the presence of neurological involvement in Behçet's disease and may lead the clinician to make an earlier diagnosis of these complications in young male patients with high risk.

    Adrenomedullin Has No Effect on Segmental Bone Defect Healing but Increases Bone Mineral Density in Rat Model

    Objective: This study aimed to investigate the effect of adrenomedullin on the healing of the segmental bone defect in a rat model. Methods: Thirty-six Wistar rats were randomly divided into 6 groups based on follow-up periods and administered a dose of adreno-medullin hormone. In each group, bilaterally, a 2-mm bone defect was created at the diaphysis of the radius. Sodium chloride solution was administered to sham groups 3 times a week for 4 and 8 weeks intraperitoneally. Adrenomedullin was administered to the study groups 3 times a week: 15 μg—4 weeks, 15 μg—8 weeks, 30 μg—4 weeks, and 30 μg—8 weeks, respectively. After euthanasia, the segmental defects were evaluated by histomorphometric [new bone area (NBA)] and microtomographic [bone volume (BV), bone surface (BS), and bone mineral density (BMD)] analyses. Results: Although the 4-and 8-week 15 μg administered study groups had higher NBA values than the other study and control groups, the histomorphometric analysis did not reveal any statistical difference between the control and study groups regarding NBA (P >.05). In microtomographic analysis, BV was higher in the 15 μg 4-week group than 30 μg 4-week group (296.9 vs. 208.5, P =.003), and BS was lower in the 30 μg 4-week group than in the 4-week control group (695.5 vs. 1334.7, P =.005), but overall, no significant difference was found between the control and study groups (P >.05). Despite these minor differences in histomorphometric and microtomographic criteria indicating new bone formation, the BMD values of the 15 μg 8-week study group showed a significant increase compared with the control group (P =.001, respectively). Conclusion: Adrenomedullin positively affected BMD at 15 μg, but this study could not show healing in the segmental defect site at different dose regimens. Further studies are needed to assess its effects on bone tissue trauma. © 2023, AVES. All rights reserved.

    Effects of Vertebral Fusion on Levels of Pro-Inflammatory and Catabolic Mediators in a Rabbit Model of Intervertebral Disc Degeneration

    Objective: The aim of this study was to explore the alterations in levels of pro-inflammatory and catabolic mediators following vertebral fusion in a rabbit model of intervertebral disc degeneration. Methods: In this study, 24 female New Zealand albino rabbits (aged 4 to 5 months and weighing 3 to 3.5 kg) were used. All the animals were randomly categorized into four groups, and dorsal spinal exposure of all lumbar vertebrae was routinely performed in each group. While disc degeneration was created in groups B, C, and D, spinal fusion was added to disc degeneration in groups C and D. Disc degeneration was typically created by puncturing the discs with an 18-gauge needle under the guidance of C-arm imaging. Fusion was achieved with posterior/posterolateral decortication and iliac bone grafts. The rabbits in groups A, B, and C were euthanized, and the discs were removed in the first week after the surgery. The rabbits in Group D were sacrificed, and the discs were harvested at 5 weeks after the surgery. The levels of Interleukin (IL)-1 beta, IL-6, Nitric Oxide (NO), Matrix Metalloproteinase (MMP)-3, MMP-13, and Tissue Inhibitor of Metalloproteinases-1 (TIMP-1) in the discs were analyzed using enzyme-linked immunosorbent assay kits. Results: Significant increase was observed in the protein levels of both pro-inflammatory and catabolic mediators in disc degeneration groups (Group B, C, and D) compared to Group A. In the fusion groups (Group C and D), these increased mediators decreased, compared to non-fusion group (Group B), (IL1-beta P = 0.017, TIMP-1 P = 0.03, NO P = 0.03). However, there was no statistically significant difference in mediator levels between the short- and long-term fusion (Group C versus D). Conclusion: The results of this study have shown that a significant decrease in pro-inflammatory and catabolic mediators may be expected after vertebral fusion whereas there may be no significant difference between the first and fourth week of fusion surgery. These findings may contribute to clarifying the mechanism of action of vertebral fusion in the treatment of low back pain.

    Effects of Coronavirus Disease-2019 Pandemic and New Normal on the Orthopedic Outpatient Clinic Practice

    Objective: This study aimed to evaluate the effects of the coronavirus disease-2019 pandemic on the orthopedic outpatient clinic, including an investigation of the number, changes in complaints, and demographics of patients. Materials and Methods: Data of patients who visited the orthopedic outpatient clinic at our hospital within four periods were evaluated. Period 1 was between March and May 2019, period 2 was between June and August 2019, period 3 was between March and May 2020, and period 4 was between June and August 2020. Data of patients were recorded for each period including age, gender, number, and International Classification of Diseases 10th Revision codes for comparative analysis. Results: This study included 7,120 patients who were admitted to the orthopedic outpatient clinic within the four periods. The lowest number of patients (1,119) who visited the outpatient clinic was observed in the three months following the pandemic declaration (period 3), in March 2020, whereas the highest number (2,149) was observed in the three months following the lifting of restrictions, in June 2020 (period 4). No significant difference was observed in the age between the four periods (p=0.945). However, a difference was found between the periods for patients aged >= 65 years. Reasons for admission were also different between the periods. In period 3, admissions due to traumatic reasons were statistically higher compared with the other three periods (p<0.001, p=0.003, p<0.001). Conclusion: It was observed that the number of patients visiting the orthopedic outpatient clinic increased after the restrictions were lifted. Outpatient clinic management must be organized in a way that prevents contamination and preserves efficient outpatient support.
