Reduction with procedural sedation and analgesia (PSA) and followed by plaster/splint are the treatment of choice. We aimed to determine the factors affecting the satisfaction in patients with DRF undergoing PSA. Methods: This prospective, observational, cross-sectional study included 70 patients with DRF. The socio-demographic features, comorbidities, level of satisfaction with PSA procedure, physical factors of the environment, physician and patient satisfaction were evaluated. PSA satisfaction scores “1, 2 and, 3” were grouped as “dissatisfied group” and “4-5” points as “satisfied group” with the Likert scale. Patient satisfaction was compared between the groups according to the satisfaction levels. Results: The median satisfaction level was found 4 (interquartile range 4-5). Their satisfaction with the given information about the PSA procedure and the cleanliness of the area where the procedure was performed was higher in the satisfied group than the dissatisfied group (p=0.014 and p=0.007, respectively). Also, as the level of residents of emergency physicians, the satisfaction of the patients increased (p=0.025). There was no significant difference between the groups in terms of age, gender, educational status, comorbidities, fracture type, additional injury, selected sedo-analgesic drugs, Richmond Agitation Sedation Scale and, complications (p>0.05). Satisfaction was high in all physicians. Conclusion: PSA procedure was satisfactory by a majority and can be performed safely in the ED. The residency period of the physician who performed the PSA, satisfaction with the given information about PSA and the cleanliness of the area were affecting the patient satisfaction.Article Türk Erişkin Nöroloji Uzmanlarında Tükenmişlik(Galenos Publ House, 2023-09-15) Uysal, Hasan Armagan; Keskin, Ahmet Onur; Güllüoğlu, Halil; Yildiz, Fatma Gokcem; Sarikaya, Fatma Gokcem YildizAmaç: Tıp doktorlarında tükenmişlik hasta bakımını veya klinisyenin fiziksel performansını kötü etkileyebilir. Çalışmamızda Türk nöroloji uzmanlarındaki tükenmişlik oranını ve tükenmişlikle ilişkili faktörleri araştırmayı amaçladık. Gereç ve Yöntem: Çalışmaya Türkiye’deki nöroloji uzmanları dahil edildi. Katılımcılardan çeşitli düşünce ve deneyimler ile ilişkili 33 sorudan oluşan bir anketi doldurmaları istendi. Katılımcılar sorulara şu şekilde cevap verdiler: Kesinlikle katılmıyorum, kısmen katılıyorum, katılıyorum, kuvvetle katılıyorum, kesinlikle katılıyorum. Sorunun anlamına göre (negatif veya pozitif), verilen cevap 1-5 arasında puanlandırıldı. Puanların toplamının maksimum (165) puana bölünmesiyle tükenmişlik oranı elde edildi. Bulgular: Ortalama yaş 38,78 (±8,42) yıl, kadın/erkek oranı 461/255 olarak bulundu. Ortalama tükenmişlik oranı %46,73 (±8,95) idi. Erkek cinsiyet, akademisyenlik, akademik derecenin yüksek olması, tıp fakültesi hastanesinde çalışmak, düşük maaş, bekar olmak, ebeveyn olmamak, nöbet tutmak, icapçı olmamak veya yoğun bakım ünitesinde çalışmak daha yüksek tükenmişlik oranı ile ilişkili bulundu. Tükenmişlik oranı, yaş, muayene edilen hasta sayısı ve çalışma saatleri ile pozitif, yardımcı sağlık personeli veya nörolog sayısı ile negatif korelasyon içindeydi. Sonuç: Bizim çalışmamız, Türk erişkin nöroloji uzmanlarından oluşan büyük bir örneklemde yüksek tükenmişlik oranını gösteren ilk çalışmadır. Erkek olmak, ileri yaş, akademisyen, profesör veya bekar olmak, ebeveyn olmamak, tıp fakültesi hastanesinde veya yoğun bakım ünitesinde çalışmak, düşük maaş, nöbet tutmak, yüksek hasta sayısı veya çalışma saati tükenmişlikle ilişkili görünmektedir.Article Cardiac Functions and Peripheral Arterial Stiffness in Patients With Polycystic Ovary Syndrome: a Cross-Sectional Study(Galenos Publ House, 2023-08-01) Colak, Ayse; Ozpelit, Mehmet Emre; Okyay, Emre; Kumral, Zeynep; Özpelit, Ebru; Okyay, Recep EmreBACKGROUND/AIMS: Polycystic ovary syndrome (PCOS) patients have been described as having subclinical cardiac and vascular damage; nevertheless, research data is contradictory. We aimed to assess global cardiac functions, peripheral arterial stiffness (AS), and the relationships between echocardiographic and AS measurements in patients with PCOS. MATERIALS AND METHODS: We enrolled 42 consecutive PCOS patients and 32 age- and body mass index (BMI)-matched healthy controls. All participants underwent a comprehensive two-dimensional echocardiographic examination. Applanation tonometry was utilized to determine peripheral AS [carotid-radial pulse wave velocity (PWV) and augmentation index (AIx)] in each participant. In addition, we evaluated the correlation between AS and echocardiographic parameters. RESULTS: The PCOS and control groups had similar ages and BMIs. Right ventricular (RV) and left ventricular (LV) diameters, LV mass, and LV ejection fraction were similar between the groups. Considering the pulse wave and tissue Doppler parameters of the cardiac functions, the LV septal S’, LV Tei index, RV S’, RV Tei index, and E/E’ ratio were comparable between the two groups. Peripheral AS parameters including, PWV and AIx were higher in those patients with PCOS [19.3±12.5 vs. 12.5±9.6; p=0.01 and 5 (4.7-5.5) vs. 4.4 (4.2-4.8); p=0.0001, respectively]. AS parameters were not correlated with echocardiographic parameters. CONCLUSION: Despite normal echocardiographic LV and RV functions, women with PCOS had increased AS. There was no correlation between echocardiographic and AS parameters in these patients.Article Citation - WoS: 1Citation - Scopus: 1Comparison of Facial Palsy Cases Before and During the Pandemic Coronavirus Disease-2019(Galenos Publ House, 2023-06-23) Uysal, Hasan Armağan; Güllüoğlu, HalilObjective: The pandemic coronavirus disease-2019 (COVID-19) is caused by a novel type of coronavirus named severe acute respiratory syndrome coronavirus-2 and is rapidly spreading all over the world. In addition to various neurological symptoms, acute facial palsy was diagnosed as the main neurological symptom in some COVID-19 patients. The current study aimed to analyze the variation and any possible association in the case numbers or medical symptoms of patients with facial palsy before and during the COVID-19 pandemic.Methods: The clinical files of patients who were diagnosed with facial palsy in the Neurology Department of Medicalpoint Hospital, University of Economics Faculty of Medicine, Izmir were retrospectively investigated. To compare the facial palsy cases according to different periods, two patient groups were formed: before the COVID-19 pandemic and during the COVID-19 pandemic. The pandemic group was further divided into two subgroups as COVID-19-positive and COVID-19-negative patients to compare the effects of COVID-19 on facial palsy.Results: During the specified COVID-19 period (May 2020-January 2021) of the study, 38 patients were admitted to the hospital for facial palsy; 34 facial palsy patients were admitted in the same calendar period as the two previous years (May 2018-January 2019). There was no significant difference in the frequency of facial palsy between these two time periods. There were significant differences between before and during the COVID-19 pandemic groups regarding response to cortisone therapy (p<0.001), facial palsy grade (p<0.001), electromyography findings (p=0.005), denervation (p<0.001), and 6 months recovery (p<0.001) data. There were also significant differences between the COVID-19-positive and COVID-19-negative subgroups regarding response to cortisone therapy (p=0.015) and facial palsy grade (p=0.001).Conclusion: The current study findings support the possible association between the severity of the clinical course of facial palsy and COVID-19. Further studies are needed to prove a direct association between facial palsy and COVID-19.Article Citation - WoS: 6Citation - Scopus: 10Comparison of Bpap S/T and Average Volume-Assured Pressure Support Modes for Hypercapnic Respiratory Failure in the Emergency Department: a Randomized Controlled Trial(Galenos Publ House, 2021-09-22) Gören, Nurfer Zehra; Sanci, Emre; Coskun, Feride Fulya Ercan; Gursoylu, Duygu; Bayram, BasakBackground: There is limited research into the utility of average volume-assured pressure support (AVAPS), a volume-assured pressure-controlled mode, especially in patients with hypercapnic respiratory failure. Aims: This study aimed at a randomized comparison of AVAPS and bilevel positive airway pressure spontaneous/timed (BPAP S/T) modes in non-invasive mechanical ventilation (NIMV) application with hypercapnic respiratory failure patients in the emergency department (ED). Study Design: Randomized controlled study. Methods: In this prospective randomized controlled study, 80 patients admitted to ED with hypercapnic respiratory failure requiring NIMV were randomly assigned to AVAPS or S/T groups using the sealed envelope method (33 patients in the S/T group, 47 patients in the AVAPS group). Data of arterial blood gas (ABG), vital parameters, Glasgow Coma Score (GCS), additional treatment needs, and clinical outcomes were evaluated, and the treatment success rates of both groups were compared. Results: A total of 80 patients, 33 in the S/T and 47 in the AVAPS group, were analyzed in the study. The pH values improved in the AVAPS group compared to the baseline (0.07 [0.04-0.10] vs 0.03 [0.00-0.11]). PaCO2 (partial pressure of carbon dioxide) excretion was faster in the AVAPS group than in the S/T group in the first hour (10.20 mmHg [6.20-19.20] vs. 4.75 ([-] 0.83-16.88)). The comparison of blood gas measurements showed no significant differences between the groups regarding the changes in PaCO2 and pH values over time (P=.141 and P=.271, respectively). During the ED follow-up, 3 (6.4%) patients in the AVAPS group and 5 (15.2%) patients in the S/T group needed intubation [Relative risk: 0.42 (95% CI: 0.11 to 1.64), P=.21]. Conclusion: In this study, improvements in blood gas parameters in the AVAPS group were faster compared to the S/T group; however, we did not find any significant difference between the groups in terms of clinical parameters.
