    Simultaneous Live Donor Liver Transplantation, Aortic Valve Replacement, and Atrial Septal Defect Repair in a Patient With End Stage Liver Disease: a Case Report

    Valvular heart disease creates an important barrier for orthotopic liver transplantation in patients with end-stage liver disease and increases mortality. Selection of the appropriate surgical scheme and adequate postoperative management can be lifesaving in these cases. This study presents a 32-year-old man diagnosed with hepatitis C-associated cirrhosis and severe aortic regurgitation due to subacute bacterial endocarditis. Initially, simultaneous aortic valve replacement (AVR) and live donor liver transplantation (LDLT) was planned. However, intraoperative transesopha- geal echocardiography revealed an additional atrial septal defect (ASD) and AVR, ASD repair, and LDLT surgery were performed. During the 2-year follow-up period, there were no early or late complications. To the best of our knowledge, this is the first patient to have simultaneous AVR, ASD repair, and LDLT surgery. Additionally, the present case is also unique in being the first person in the Republic of Azerbaijan to undergo concomitant cardiac surgery and LDLT.

    Crigler-Najjar sendromu, unkonjüge hiperbilirubinemiye neden olan nadir, kalıtsal bir hastalıktır. Karaciğer nakli, Crigler-Najjar sendromu için kesin tedavi sağlayan bir seçenektir. Bu olgu sunumunda Crigler-Najjar sendromu nedeniyle karaciğer nakli yapılan iki hasta sunuldu. Gilbert sendromu olarak yanlış teşhis konulan ilk hasta 15 yaşında bir erkekti. Var olan konuşma ve yürüme bozukluğu karaciğer nakli sonrası kısmi olarak düzeldi. İkinci hasta 22 yaşında erkekti. Hastalığın hafif bir klinik formuna sahip olmasına rağmen, karaciğer fibrozisi gelişmişti. Karaciğer nakli her iki hastaya da önemli bir morbidite olmaksızın başarıyla uygulandı.

    The Expression of Forkhead Transcription Factors in Decidua and Placenta in Patients With Missed Abortion

    Background: Forkhead transcription factors 3a (FOXO3a) has pleiotropic biological functions in the female reproductive tract. FOXO3a has a function in decidualization, in placental development, and also in inhibition of apoptosis. This study aims to investigate a possible role of FOXO3a in missed abortion. Materials and Methods: Decidual and placental tissue samples were obtained from the women with unwanted pregnancy as the control group and with missed abortion as the patient group. Immunohistochemistry technique was utilized to compare FOXO3a expression of the decidual cells in uterine decidual stroma and cytotrophoblast-syncytiotrophoblast cells in placental villous stroma. Immunohistochemistry was evaluated semi-quantitatively utilizing the H-score technique. Results: It was demonstrated that H-Scores of FOXO3a expression in both uterine decidual stroma were increased in the missed abortion group (255.83 +/- 12.41) than in the normal pregnancy group (133.33 +/- 17.43). It was also shown that there was no difference between non-decidual area of the endometrium of the normal pregnancy and the missed abortion group (30.33 +/- 4.32; 39.66 +/- 14.30, respectively) and placental villous stroma (13.00 +/- 1.89; 13.00 +/- 1.67, respectively). However, the immunoreactivity of cytotrophoblast and syncytiotrophoblast cells significantly increased in the missed abortion group (18.83 +/- 1.47; 322.00 +/- 6.06, respectively) than in the normal pregnancy group (11.00 +/- 1.26; 254.00 +/- 8.17, respectively) (p < 0.05). Conclusion: These data support the hypothesis that increased FOXO3a expression in missed abortion may prevent the discharge of dead fetus to maintain decidualization, prevention of oxidative stress, immunomodulation, and inhibition of apoptosis.

    Laparoskopik Adrenalektomi Sonrası Hastanede Kalış Süresine Etkili Faktörler

    Amaç: Bu çalışmanın amacı laparoskopik adrenalektomi uygulanan hastalarda, ameliyat sonrasında hastanede kalış süresine etki eden faktörleri değerlendirmektir. Yöntem: 2012-2022 yılları arası laparoskopik adrenalektomi uygulanan hastalar retrospektif olarak incelendi. Hastanede kalış süresi için çalışma popülasyonunun 75. yüzdelik dilimi sınır olarak belirlendi. Bu süreden daha uzun süre kalınması uzamış hastanede kalış olarak tanımlandı. Uzamış hastanede kalışa etkili faktörler tek değişkenli ve çoklu lojistik regresyon analizleriyle incelendi. Tek değişkenli lojistik regresyon analizinde p-değeri <0,10 olan değişkenler çoklu regresyon modeline dahil edildi. Bulgular: Çalışmaya toplam 86 hasta dahil edildi. Hastaların 71’i (%83) kadındı. Ortanca yaş 54 (çeyrekler arası aralık, 45-61) yıldı. Ortanca hastanede kalış süresi 2 (interkuartil aralık, 2-2) gündü. Popülasyonun 75. yüzdelik dilimine göre 2 günden uzun süren kalış süresi uzamış hastanede kalış olarak belirlendi. Buna göre 14 (%16) hastada uzamış hastanede kalış saptandı. Çoklu regresyon analizi sonucunda operasyon süresi (olasılık oranı: 1,03; p-değeri: 0,017), açık cerrahiye geçiş (odds oranı: 4,79; p-değeri: 0,045) ve erkek cinsiyet (olasılık oranı: 5,61; p-değeri: 0,022), uzamış hastanede kalış süresi ile ilişkili bulundu. Sonuç: Laparoskopik adrenalektomi, bir çok adrenal patolojinin tedavisinde güvenilir bir yöntemdir. Operasyon süresi ve açık cerrahiye geçiş, uzamış hastanede kalış süresi ile ilişkili bulunmuştur; ancak kanıt düzeyi yüksek çalışmalara ihtiyaç vardır.

    Prediction of Short-Term Results After Laparoscopic Right Hemicolectomy. Is Sarcopenia Superior To Other Methods?

    Objective: We aimed to determine the factors that predict early mortality and morbidity in patients who underwent laparoscopic right hemicolectomy and their superiority over each other. Method: Demographic data, age -adjusted Charlson Comorbidity Index, American Society of Anesthesiologists Score, body mass index, modified -Glasgow Prognostic Score (mGPS), stage of disease, and sarcopenia were evaluated in patients who underwent right hemicolectomy between 2010-2022. Their superiority in predicting short-term outcomes was compared. Results: 78 patients were included in the study. The complication rate was higher in sarcopenic patients (p = 0.002). A high mGPS score was associated with increased mortality risk (p = 0.012). Other methods were not found to be related to short-term results. Conclusions: Sarcopenia is useful for the prediction of complications, and the mortality rate can be estimated by the mGPS score. These are superior to the other short-term results prediction methods. However, randomized controlled studies are needed.

    A Comparison of the Molecular Distribution of Proangiogenic Factors in Endometrium of Missed Abortions and of Voluntary First Trimester Termination Cases

    Objective: The authors aimed to evaluate the angiogenic changes that occur in the cases with missed abortions compared with the voluntary termination of pregnancy as control group, with this controlled clinical study. Materials and Methods: The study included fifteen healthy volunteer women with unwanted pregnancy less than 10th gestational week in an academic research environment. The patients were 19 women between 6th and 11th gestational weeks diagnosed with missed abortion as the patient group. Immunohistochemistry was utilized to examine temporal and spatial expression of vascular endothelial growth factor (VEGF) and their two receptors: VEGF-Rl (Flt-1) and VEGF-R2 (Flk-1/KDR), and Trombospondin-1, eNOS, iNOS, and HIF-1 alpha in the both deciduas and placenta of the both groups. Results: This study discovered the significant difference (p < 0.005) between the groups of controlled and missed abortion in the decidual and placental cell components, and has put forward that thrombospondin and iNOS have an impact on abortion through antiangiogenic effect in cases of missed abortions. Conclusions: The potential role of molecules affecting angiogenesis in the etiology of missed abortion has been evaluated and the authors aimed for this to be a guide for studies on further treatments and on the prevention of the development of missed abortions.

    Concurrent Living Donor Liver Transplantation and Off-Pump Coronary Artery Bypass in a Five-Year Child With Homozygous Familial Hypercholesterolemia: a Case Report

    Background: Homozygous familial hypercholesterolemia (FH) is a rare disease that causes serious cardiovascular problems and may be fatal even at an early age. Because this disease variant is rather aggressive, the effect of lipid-lowering agents and lipid apheresis remains inadequate in most cases. In patients who are not responding or tolerating available treatments, liver transplantation (LT) is the last and definitive solution. In addition to LT, the need for cardiac surgery is often substantial. Case report: This study presents the first pediatric case of FH who underwent off-pump coronary artery bypass (OPCAB) surgery concurrent with live-donor LT (LDLT). The early postoperative period was uneventful. After a 1-year follow-up period, the patient is alive and well with normal liver functions and cholesterol levels remaining within the normal range without any lipid-lowering medical therapy. Conclusion: Performing simultaneous coronary bypass and LT may be safe and feasible even for pediatric patients. © 2023 Elsevier Inc.

    Evaluation of the Relationship Between Prognostic Nutritional Index and Early Clinical Outcomes in Intestinal Transplantation

    Background: Malnutrition in patients with intestinal insufficiency negatively affects the success of intestinal transplantation. In our research, we assessed patients' nutritional status using the PNI scale and examined the impact of the PNI score on clinical outcomes during the post-transplant phase. Results: The acute rejection rate was 38.1%, and the 30-day survival rate was 90.5%. Median PNI values were 41.5 (min-max: 33.5-65), 29.5 (min-max: 13.5-56.5), 33 (min-max: 3-51), 35.7 (min-max: 24.5-54), 33.5 (min-max: 24.5-75.5) preoperatively and on postoperative 1, 7, 15, and 30 days, respectively. No significant relationship was found between the other parameters and the PNI. Conclusions: The PNI score alone fails to adequately represent a patient's nutritional status and has not demonstrated effectiveness in predicting early-term outcomes for those undergoing intestinal transplantation. Additional studies involving a larger number of patients and diverse nutritional markers are necessary for further insights into this matter. © 2025 Elsevier B.V., All rights reserved.
