Reduction with procedural sedation and analgesia (PSA) and followed by plaster/splint are the treatment of choice. We aimed to determine the factors affecting the satisfaction in patients with DRF undergoing PSA. Methods: This prospective, observational, cross-sectional study included 70 patients with DRF. The socio-demographic features, comorbidities, level of satisfaction with PSA procedure, physical factors of the environment, physician and patient satisfaction were evaluated. PSA satisfaction scores “1, 2 and, 3” were grouped as “dissatisfied group” and “4-5” points as “satisfied group” with the Likert scale. Patient satisfaction was compared between the groups according to the satisfaction levels. Results: The median satisfaction level was found 4 (interquartile range 4-5). Their satisfaction with the given information about the PSA procedure and the cleanliness of the area where the procedure was performed was higher in the satisfied group than the dissatisfied group (p=0.014 and p=0.007, respectively). Also, as the level of residents of emergency physicians, the satisfaction of the patients increased (p=0.025). There was no significant difference between the groups in terms of age, gender, educational status, comorbidities, fracture type, additional injury, selected sedo-analgesic drugs, Richmond Agitation Sedation Scale and, complications (p>0.05). Satisfaction was high in all physicians. Conclusion: PSA procedure was satisfactory by a majority and can be performed safely in the ED. The residency period of the physician who performed the PSA, satisfaction with the given information about PSA and the cleanliness of the area were affecting the patient satisfaction.Article Differences Between Patients With and Without Persistent PSA After Radical Prostatectomy in Clinically High-Risk And/Or Locally Advanced Prostate Cancer(Galenos Publ House, 2025-02-24) Eker, Anıl; Degirmenci, Tansu; Bozkurt, İbrahim Halil; Celik, Serdar; Gunlusoy, Bulent; Çetin, Taha; Karaca, Berk; Cinar, Mahmut; Dagasan, Muhammet Halil; Muratoglu, MurathanObjective: This study aimed to identify preoperative and postoperative factors associated with persistent prostate-specific antigen (PSA) following radical prostatectomy (RP) in patients with clinically high-risk and/or locally advanced prostate cancer. Understanding these factors can guide early postoperative management decisions, including adjuvant treatment strategies. Materials and Methods: A retrospective analysis was conducted on 183 patients who underwent RP for locally advanced prostate cancer between 2009 and 2023. Patients were divided into two groups: those with persistent PSA at 1 month postoperatively (group 2, n=43), and those without (group 1, n=140). Preoperative and postoperative variables, including PSA levels, clinical stage, biopsy grade group, tumor volume, and pathological findings, were compared between groups. Results: Patients in group 2 had significantly higher preoperative PSA levels (24.6±19 ng/mL vs. 15±15.5 ng/mL, p<0.001), advanced clinical stage (≥T2B: 52.6% vs. 32.1%, p=0.032), and higher percentage of positive biopsy cores (p=0.011). Postoperative findings demonstrated a higher tumor volume (20.2±14.1 cc vs. 10.7±10.5 cc, p=0.002), tumor density (p=0.005), and positive surgical margins (86% vs. 70%, p=0.025) in group 2. Patients in group 2 had higher rates of lymph node dissection, adjuvant therapy, and early salvage radiotherapy. Conclusion: Preoperative PSA levels, biopsy grade group, positive surgical margins, and advanced pathological stage are critical predictors of persistent PSA after RP. Early identification of high-risk patients enables personalized management plans, including timely initiation of adjuvant therapies, to improve outcomes. Further prospective studies are needed to refine risk stratification models and personalize treatment strategies.Article Which Measures and Parameters of Heart Rate Variability Analysis May Be Useful for Early Detection and Predicting Prognosis of Sepsis? a Systematic Review(Galenos Publ House, 2025-02-26) Ozel, Hasan Fehmı; Kazdağlı, HasanSepsis, kardiyovasküler, solunum ve termoregülasyon gibi sistemlerde patolojik değişikliklere neden olur. Bu değişiklikler de kalp hızı değişkenliğinde (HRV) alterasyonlara neden olur. Vital bulgularda veya hastalığın klinik sunumunda herhangi bir değişiklik olmasa bile, enfeksiyona bağlı olarak sempatik sinir sistemi aktivasyonu nedeniyle HRV parametreleri değişebilir. Bu sistematik derlemedeki amacımız, literatürü inceleyerek sepsise ilişkin HRV ölçümlerini ve parametrelerini sunmak ve bunların sepsisin şiddetini ve ölüm riskini tahmin etmedeki olası rolünü araştırmaktır. Veritabanları, Nisan 1996 - Mayıs 2023 tarihleri arasında İngilizce dilinde yayınlanmış sepsis üzerine HRV analizlerini insan çalışmalarını bildiren orijinal araştırma makaleleri için tarandı. Makale araması tamamlandıktan sonra, 79 makale daha ayrıntılı bir değerlendirmeye tabi tutulmak üzere seçildi ve bu makalelerin tam metinleri incelendikten sonra 13 makale kriterlere uygun olarak sınıflandırıldı. Her HRV parametrelerinin ortalama değerleri her çalışmanın örnek büyüklüğüne göre düzeltildi ve genel ortalamalar hesaplandı. İstatistiksel karşılaştırmalar Wilcoxon eşleştirilmiş diziler testi ile yapıldı. Toplam 1453 hastanın yer aldığı dokuz çalışma dahil edildi, ortalama yaş 64,24 yıl ve tüm katılımcıların %53,9’u erkekti. Dahil edilen çalışmaların hepsi zaman, frekans domain analizi gerçekleştirdi ve dört tanesi bu analizlere ek olarak doğrusal olmayan analizler gerçekleştirdi. Dokuz çalışmanın yedisini acil serviste ve ikisini hastanelerin yoğun bakım ünitelerinde gerçekleştirildi. Altı çalışma sağ kalanlar ile hayatını kaybedenler arasındaki parametreleri, üç çalışma ise sepsisin farklı şiddet seviyeleri arasındaki parametreleri karşılaştırdı. SDNN, RMSSD, SDNN, HTI, LFnu, HFnu, LF/HF oranı, SD1, SD2, eğilimsiz dalgalanma analizi (DFA)α1 ve DFAα2, sepsis sonucuyla ilişkili gibi görünmektedir. Bu nedenle, sepsisin erken teşhisi için bu parametrelerin izlenmesinin faydalı olabilir.Article Bioinformatics Based Drug Repurposing Approach for Breast and Gynecological Cancers: RECQL4/FAM13C Genes Address Common Hub Genes and Drugs(Galenos Publ House, 2025-01-02) Duran, Gizem Ayna; Duran, Assist. Prof. Dr. Gizem AynaObjective: The prevalence of breast cancer and gynaecological cancers is high, and these cancer types can occur consecutively as secondary cancers. The aim of our study is to determine the genes commonly expressed in these cancers and to identify the common hub genes and drug components. Materials and Methods: Gene intensity values of breast cancer, gynaecological cancers such as cervical, ovarian and endometrial cancers were used from the Gene Expression Omnibus database Affymetrix Human Genome U133 Plus 2.0 Array project. Using the linear modelling method included in the R LIMMA package, genes that differ between healthy individuals and cancer patients were identified. Hub genes were determined using cytoHubba in Cytoscape program. “ShinyGo 0.80” tool was used to determine the disease-specific biological KEGG pathways. Drug.MATADOR from the ShinyGo 0.80 tool was used to predict drug-target relationships. Results: The RecQ Like Helicase 4 and Family with Sequence Similarity 13 Member C genes were found to be similarly expressed in breast cancer and gynaecological cancers. Upon KEGG pathway analyses with hub genes, Drug.MATADOR analysis with hub genes related to cancer related pathways was performed. We have determined these gene/drug interactions: NBN (targeted by Hydroxyurea), EP300 (targeted by Acetylcarnitine) and MAPK14 (targeted by Salicylate and Dibutyryl cyclic AMP). Conclusion: The drugs associated with hub genes determined in our study are not routinely used in cancer treatment. Our study offers the opportunity to identify the target genes of drugs used in breast and gynaecological cancers with the drug repurposing approach.Article Türk Erişkin Nöroloji Uzmanlarında Tükenmişlik(Galenos Publ House, 2023-09-15) Uysal, Hasan Armagan; Keskin, Ahmet Onur; Güllüoğlu, Halil; Yildiz, Fatma Gokcem; Sarikaya, Fatma Gokcem YildizAmaç: Tıp doktorlarında tükenmişlik hasta bakımını veya klinisyenin fiziksel performansını kötü etkileyebilir. Çalışmamızda Türk nöroloji uzmanlarındaki tükenmişlik oranını ve tükenmişlikle ilişkili faktörleri araştırmayı amaçladık. Gereç ve Yöntem: Çalışmaya Türkiye’deki nöroloji uzmanları dahil edildi. Katılımcılardan çeşitli düşünce ve deneyimler ile ilişkili 33 sorudan oluşan bir anketi doldurmaları istendi. Katılımcılar sorulara şu şekilde cevap verdiler: Kesinlikle katılmıyorum, kısmen katılıyorum, katılıyorum, kuvvetle katılıyorum, kesinlikle katılıyorum. Sorunun anlamına göre (negatif veya pozitif), verilen cevap 1-5 arasında puanlandırıldı. Puanların toplamının maksimum (165) puana bölünmesiyle tükenmişlik oranı elde edildi. Bulgular: Ortalama yaş 38,78 (±8,42) yıl, kadın/erkek oranı 461/255 olarak bulundu. Ortalama tükenmişlik oranı %46,73 (±8,95) idi. Erkek cinsiyet, akademisyenlik, akademik derecenin yüksek olması, tıp fakültesi hastanesinde çalışmak, düşük maaş, bekar olmak, ebeveyn olmamak, nöbet tutmak, icapçı olmamak veya yoğun bakım ünitesinde çalışmak daha yüksek tükenmişlik oranı ile ilişkili bulundu. Tükenmişlik oranı, yaş, muayene edilen hasta sayısı ve çalışma saatleri ile pozitif, yardımcı sağlık personeli veya nörolog sayısı ile negatif korelasyon içindeydi. Sonuç: Bizim çalışmamız, Türk erişkin nöroloji uzmanlarından oluşan büyük bir örneklemde yüksek tükenmişlik oranını gösteren ilk çalışmadır. Most MS cases occur sporadically, but a considerable proportion of patients have a family history of MS. The etiology and pathophysiology of MS remain unclear. Recent epidemiological and gene expression studies have indicated that dysregulation of microRNAs (miRNAs) may play a role in MS pathogenesis. This study aimed to evaluate the differential expression of miRNAs in sporadic MS (sMS) and familial MS Materials and Methods: This cross-section, single-center study was conducted in 20 FMS and 10 sMS patients and 8 healthy controls. The patients were in the remission. In total, 2,549 miRNA genes were screened in the blood mononuclear cells from the whole blood samples of MS patients depending on miRBase 21. Differential expression of miRNAs in MS patients was identified compared with the control group, and miRNAs with a fold change >= 2 were validated using reverse transcription-polymerase chain reaction. Differentially expressed miRNAs were then compared between FMS and sMS patients. Results: Initial findings showed that miR-5100 and hsa-miR-16-2-3p were increased and miR-432-3p was decreased in FMS compared with sMS, whereas miR-548-aa, hsa-miR-142-3p, and miR-451-b were increased in both sMS and FMS, but miR-548-v was increased only in sMS. Some miRNAs showed the same expression patterns in both groups. Conclusion: Differential expression of certain miRNAs may be a useful biomarker in the diagnosis of MS. This study showed that miRNAs may discriminate between FMS and sMS cases and MS subtypes, as indicated in earlier studies.Article New Lymph Node Parameters and a Comparison With the American Joint Committee on Cancer N-Stages in Breast Cancer(Galenos Publ House, 2023-08-01) Eliyatkin, Nuket Ozkavruk; Başkır, İnci; İşlek, Akif; Zengel, BahaBACKGROUND/AIMS: The N-stage of TNM systems considers only the number of metastatic lymph nodes (NMLN) in breast cancer (BC). However, new lymph node parameters refer to the number of harvested lymph nodes (NHLN) and negative lymph nodes (NNLN), which have had an increasing significance in the current literature. This study aimed to compare NHLN, NNLN, lymph node ratio (LNR), modified lymph node ratio (mLNR), and log odds of positive lymph nodes (LODDS) against the standard American Joint Committee on Cancer (AJCC) N-stage for the prognosis of BC patients. MATERIALS AND METHODS: This study was designed retrospectively. The socio-demographic data, clinical features, histopathological factors, treatment modalities, receptor status of BC, and lymph node related parameters (AJCC N, LNR, mLNR, LODDS) were identified. Then, lymph node related parameters were compared for cancer-related mortality (CRM), cancer recurrence, disease-free survival (DFS), and overall survival (OS). RESULTS: Eight hundred seven women who underwent surgery for BC were included in this study according to its eligibility criteria. The mean follow-up period was 113.34±74.85 (range: 6-378) months. The NHLN was 21.24±9.22, the NMLN was 4.85±7.38, the NNLN was 16.39±9.48, the LNR was 0.23±0.29, the mLNR was 5.38±7.38 and the LODDS was -0.74±0.80 on average. During the follow-up period, 42 (5.2%) patients had local recurrence, 188 (23.3%) had distant metastases, and 252 (31.2%) patients died due to BC. NMLN, LNR, mLNR, and LODDS were found to be significantly higher, and NNLN was significantly lower in those patients with cancer recurrence and CRM (p<0.001). AJCC N-stages, and also LNR, mLNR, and LODDS groups according to the calculated cut-off values, were significant for DFS and OS according to survival analysis. In Cox regression analysis, only LODDS was a significant independent risk factor for OS [p=0.014, heart rate (HR)=3.78, 95% confidence interval (CI) for HR: 1.30-10.94)]. CONCLUSION: The results indicated that LODDS was more successful compared to other lymph node staging systems, especially for OS. However, randomized prospective controlled studies with larger samples and homogeneous study groups are needed to create standard classification systems as alternatives to AJCC N.Article Cardiac Functions and Peripheral Arterial Stiffness in Patients With Polycystic Ovary Syndrome: a Cross-Sectional Study(Galenos Publ House, 2023-08-01) Colak, Ayse; Ozpelit, Mehmet Emre; Okyay, Emre; Kumral, Zeynep; Özpelit, Ebru; Okyay, Recep EmreBACKGROUND/AIMS: Polycystic ovary syndrome (PCOS) patients have been described as having subclinical cardiac and vascular damage; nevertheless, research data is contradictory. We aimed to assess global cardiac functions, peripheral arterial stiffness (AS), and the relationships between echocardiographic and AS measurements in patients with PCOS. MATERIALS AND METHODS: We enrolled 42 consecutive PCOS patients and 32 age- and body mass index (BMI)-matched healthy controls. All participants underwent a comprehensive two-dimensional echocardiographic examination. Applanation tonometry was utilized to determine peripheral AS [carotid-radial pulse wave velocity (PWV) and augmentation index (AIx)] in each participant. In addition, we evaluated the correlation between AS and echocardiographic parameters. RESULTS: The PCOS and control groups had similar ages and BMIs. Right ventricular (RV) and left ventricular (LV) diameters, LV mass, and LV ejection fraction were similar between the groups. Considering the pulse wave and tissue Doppler parameters of the cardiac functions, the LV septal S’, LV Tei index, RV S’, RV Tei index, and E/E’ ratio were comparable between the two groups. Peripheral AS parameters including, PWV and AIx were higher in those patients with PCOS [19.3±12.5 vs. 12.5±9.6; p=0.01 and 5 (4.7-5.5) vs. 4.4 (4.2-4.8); p=0.0001, respectively]. AS parameters were not correlated with echocardiographic parameters. CONCLUSION: Despite normal echocardiographic LV and RV functions, women with PCOS had increased AS. There was no correlation between echocardiographic and AS parameters in these patients.Article Cortical Thickness Alterations in Parkinson's Disease With Mild Cognitive Impairment(Galenos Publ House, 2023-07-03) Çavuşoğlu, Berrin; Hünerli, Duygu; Kıyı, İlayda; Çakmur, Raif; Yener, Görsev; Ada, EmelObjective: This study investigated cortical thickness differences and their relationships with cognitive functions in Parkinson's disease (PD) with mild cognitive impairment (MCI) and cognitively normal (CN). Materials and Methods: Twenty-two patients with PD-MCI, 23 with PD-CN, and 23 healthy controls with structural brain magnetic resonance imaging scans and complete neuropsychological tests were enrolled in this study. Cortical thickness analysis was performed using the Statistical Parametric Mapping 12 software package. Correlations with cognitive functions were examined. Results: Cortical thickness was significantly lower in the PD-CN and PD-MCI patient groups than in healthy controls in the left precuneus and isthmus-cingulate cortex, right pars orbitalis, insula, and lateral orbitofrontal cortex. In addition, the PD-MCI group also exhibited cortical thinning in the left superior temporal gyrus, transverse temporal cortex, supramarginal gyrus, and bilateral posterior cingulate cortex compared with healthy controls. Correlation analyses among cortical thickness and cognitive scores of PD also revealed moderate associations between memory and the posterior cingulate cortex; language and the precuneus; and executive functions and the insula and isthmus-posterior cingulate cortices. Conclusion: MCI in PD may be related to cortical alterations in the posterior cingulate cortex and the left temporoparietal cortex, which has been associated with subtle cognitive deficits in PD.
